摘　　要：目的:26例哮喘患者应用峰流速仪测定峰流速值(PEF值)、观察PEF变异率的情况,总结峰流速仪监测对支气管哮喘治疗的指导意义。方法:选择呼吸内科就诊的26例支气管哮喘患者为研究对象,连续8周每天监测呼气峰流速值,计算PEF变异率。与同期选择的非哮喘组37例作对照,比较说明PEF对肺功能的影响和临床治疗的指导价值。结果:治疗组26例与对照组37例均完成了8周呼气峰流速值的监测,哮喘组PEF值显著降低,变异率显著增高,大于20%。哮喘患者吸入β2受体激动剂后15～20分钟,PEF增高率为52%左右。本组患者吸入药物治疗8周中,PEF值逐周增高,而变异率降低,患者的症状常在4～8周内逐步缓解或控制,并随治疗时间的延长而疗效增加。结论:正确的使用峰流速仪能较科学地了解病情变化、预防发作、观察药物疗效及指导治疗。每日监测PEF值对发现在出现症状之前的哮喘发作、预防哮喘夜间猝死、发现环境中诱发因素及指导哮喘的管理和治疗均有价值。提示我们应加强对峰流速仪的推广。Objective:Observes 26 examples asthmas patient application peak current meterdeter mination peak speed of flow value namely (the PEF value), the PEF variation rate situation, summarizes the peak current meter to manage at the bronchial tube asthma the application experience. Method :The choice breath internal medicine department receives a medical examination 26 examples bronchial tubes asthma patient is the research object, continuously 8 weeks every day monitor the expiration peak speed of flow value, calculates the PEF variation rate. The non-asthma group 37 examples chooses which with the same time make the comparison, compared with explains PEF to the lung function in fluence . Result :The treatment group 26 examples and the comparison group 37 examples completed 8 weeks expiration peaks speed of flow value monitor, the asthma group PEF value remarkably reduced, the variation rate remark ably advanced, is bigger than 20%。The asthma patient inspiration beta 2 acceptors excited medicinal preparations latter 15-20 minute, PEF advancing rate is about 52%.This group of patients inspiration medicine treats in 8 weeks, the PEF value advances by the week, but the variation rate reduces, patient's symptom often gradually alleviates or the control in 4-8 week, and lengthens but the curative effect increase along with the treatment time. Conclusion :The correct use peak current meter can scientifically understand the condition change, the prevention manifests suddenly, the observationme clicine curative effect and the instruction treatment. Monitors the PEF value to discover every day in has in front of the symptom the asthma manifests suddenly, the prevention asthma at night sudden dies ,in the discovery environment induces the factor and the instruction asthma management and the treatment has the value. Prompts us to be supposed to streng then to the peak current meter promotion.
