腹腔镜子宫切除术与经腹子宫切除术的临床对照分析

摘　　要：目的 :评价腹腔镜子宫切除术的临床效果及价值。方法 :因子宫肌瘤、子宫腺肌症、子宫内膜异位症等指征实施腹腔镜子宫切除术 5 2例 (TLH组 ) ,同期行经腹子宫切除术 6 0例作为对照 (TAH组 ) ,比较两组病人术中、术后情况。结果 :与TAH组比较 ,TLH组手术时间明显延长 (P <0 0 5 ) ,术中出血量显著减少 (P <0 0 1 ) ,术后体温恢复时间、下床活动时间、住院时间均显著缩短 (P <0 0 1 )。TLH组手术损伤及术后阴道残端出血发生率无增高。TLH组术后恢复日常活动、恢复正常性生活的时间缩短。结论 :腹腔镜子宫切除术虽不能完全代替经腹子宫切除术。但创伤小、恢复快 ,是一种安全。Objective: To investigate the clinical effects and value of total laparoscopic hysterectomy (TLH) Methods: 52 cases of TLH compared with 60 cases of total abdominal hysterectomy (TAH), with both indications including uterine myomas, adenomyosis and endometrisis Results: In the cases of TLH, the average operating time were signifcantly longer and the estimated blood loss were significantly less than cases of TAH ( P <0 05) The time of temperature recovering postoperative, the time of out of bed activity postoperative and the time of and hospitalization of TLH were significantly shorter than those of TAH ( P <0 01) The intraoperative and postoperative complications in cases of TLH were not significantly increased It was quicker returning to normal activity and sexual life of cases of TLH Conclusion: Although it cannot replace TAH completely, TLH is a safe, reliable method of removing uterus and can be performed widely
