摘  要:目的 探讨神经导航系统在后颅凹手术中的应用价值 ,总结克服脑漂移的技术操作体会。方法 在36例后颅凹手术中 ,应用美国枢法莫 丹历公司的Stealthstation导航系统进行病灶定位。结果 成功切除后颅凹肿瘤 2 8例 ,夹闭动脉瘤 3例 ,切除AVM 2例 ,切除静脉畸形、进行活检和取颅内金属异物各 1例。效果满意 ,无手术死亡者 ,除 6例听神经瘤患者术后出现面瘫外 ,无其他手术并发症。结论 应用导航技术进行后颅凹病变手术 ,可以完成术中病灶的精确定位和最佳手术入路的选择 ,并可降低术中医源性损伤 。Objective To discuss the value and safety of neuronavigation in posterior cranial fossa surgery,especially the role of minimizing post operative complication.Methods A retrospective review of 36 cases of neurosurgical operation in which an image guided system supplied by Sofamor Danek company were used.From 1999~2002,36 cases of patients underwent this operation associated with navigation system,including 28 tumors,3 aneurysms,2 AVM,1 venous malformation,1 biopsy,1 metal foreign.Results There were no other post operative neurological complications except 6 facial palsy in acoustic neuroma.No operative death occured.The intracranial lesions and important anatomic structures were accurately located.Conclusions The neuronavigation system is valuable and very helpful for posterior cranial fossa operations,especially those with lesions located in the deep center of the brain.In addition,its accurate location has a signi ficant importance in protection of normal brain tissues and complete resection of lesions.
