摘　　要：为探讨新生儿急性肾功能衰竭 (ARF)血、尿胰岛素样生长因子_Ⅰ (IGF_Ⅰ)的变化及意义 ,采用放射免疫法测定17例对照组新生儿和31例ARF新生儿少尿期、多尿期和恢复期血清和尿IGF_Ⅰ水平。结果显示 ,肾衰组尿IGF_Ⅰ在少尿期和多尿期均显著低于对照组 (P均<0.001) ,恢复期后尿IGF_Ⅰ水平升至正常 ,与少尿期和多尿期比较差异均有显著性 (P<0.001 ,P<0.01) ;肾衰组恢复期尿IGF_Ⅰ与同期尿素氮和血肌酐水平呈显著负相关 (r= -0.425,P<0.05;r= -0.563 ,P<0.01) ;肾衰组血清IGF_Ⅰ显著低于对照组 ,但不同时期的ARF血清IGF_Ⅰ水平无显著变化。提示在新生儿ARF过程中尿IGF_Ⅰ呈先下降后上升变化趋势 。Insulin-like growth factor-Ⅰ(IGF-Ⅰ)is a polypeptide closely related with fetal and infant growth and development.It has been reported that IGF-Ⅰplays important roles in renal development and recovery of acute renal injury.To investigate the dynamic changes and their significance of serum and urine IGF-Ⅰin neonates with acute renal failure(ARF),the serum levels and urine IGF-Ⅰwas determined with radioimmunoassay at different time points in31neonates with ARF and17healthy controls.The results in-dicated that urine IGF-Ⅰlevels decreased significantly in ARF group at their stages of oliguria and polyuria compared to the control group.(81.16μg±21.83μg/L,92.77μg±26.50μg/L vs.123.24μg±26.10μg/L,respectively,P<0.001).The urine IGF-Ⅰreturned to normal levels at the recovery stage(109.13μg±24.16μg/L).There was a significant negative correlation between urine IGF-Ⅰand serum BUN or creatinine in ARF group at the recovery stage.(r=-0.425,P<0.05;r=-0.563,P<0.01).The serum IGF-Ⅰlevels of ARF group were significantly lower than those in control group,but no significant differences was found in different stages of ARF,suggesting that urine IGF-Ⅰlevels in neonatal ARF patients could reflect the altera- tion patterns of the renal function status,i.e.with low levels at renal failure stage and with normal levels at recovery stage.
