摘 要:目的观察糖皮质激素对骨髓基质细胞增殖及分化的作用,探讨糖皮质激素导致继发性骨质疏松的病理学机制。方法骨髓基质细胞以随机数字表法随机分为对照组和实验组。以1×10-7mol/L地塞米松加入髓基质细胞培养物中,测定增殖的骨髓基质细胞数及培养液中骨钙素含量。通过苏丹III脂肪细胞染色计数观察地塞米松作用时间对脂肪细胞分化的影响。结果实验组及培养液中骨钙素含量(5.2±1.4)ng/mg蛋白质,明显低于对照组(12.5±1.5)ng/mg蛋白质(t=12.61,P<0.001);骨髓基质细胞数实验组(3.73±0.43)×105个细胞/孔,明显低于对照组(5.76±0.69)×105个细胞/孔(t=7.45,P<0.001);脂肪细胞的数量随地塞米松作用时间延长而增多。结论糖皮质激素抑制骨髓基质细胞增殖及向成骨细胞分化,促进其向脂肪细胞分化,这可能与糖皮质激素引起继发性骨质疏松时,骨量减少,髓内脂肪组织增多有关。Aim To observe the effect of glucocorticoids on marrow stromal cell proliferation and differentiation and study the pathological mechanism of osteoporosis.Methods Bone marrow matrix cells were divided into experiment and control groups according to random number table.Proliferation of marrow stromal cell treated with dexamethasone(1×10-7 mol/L) and contents of osteocalcin in the media were measured.Results The number of the marrow stromal cell[(3.73±0.43)×105]and the contents of osteocalcin[(5.2±1.4)ng/mg]in the media of the experimental group obviously decreased compared to the control[(5.76±0.69)×105,(12.5±1.5)ng/mg](t=12.61,7.45,P< 0.001).The number of adipocytes increased with longer duration of exposure to dexamethasone.Conclusion Glucocorticoids inhibited marrow stromal cell proliferation and osteoblastic differentiation, and promoted its differentiation into adipocytes,which is a major contributing factor of bone loss and an increase in marrow fat volume in the osteoporosis.
