严重腹腔感染时丹参注射液对胃粘膜血流量和胃粘膜内pH值的影响

摘　　要：目的 探讨丹参注射液对严重腹腔感染状态下大鼠胃粘膜血流量(GMBF)和胃粘膜内pH值(pHi)的影响。方法 将大鼠随机分为对照组、感染组和丹参组;感染组、丹参组大鼠造成肠结扎穿孔致腹腔严重感染模型;丹参组给予丹参注射液。测定穿孔前和穿孔后3、6、12、24、48h各时相大鼠GMBF及pHi。结果 与对照组比较,感染组GMBF在穿孔后3h明显降低,12h降至最低,24h开始回升,48h仍低于对照组;丹参组GMBF在6、12、24h高于感染组,pHi于穿孔后3h即有下降,6h明显低于对照组,12h下降至最低,24h有所恢复,但低于对照组;丹参组pHi在12、24、48h高于感染组。结论 严重腹腔感染状态下GMBF及pHi降低可能是引起胃粘膜屏障受损的主要原因。早期应用丹参注射液对有效预防应激性溃疡的发生有重要临床意义。Objective: To investigate the effect of Dashen Injection on the changes of gastric mucosal blood flow and pH during severe intraperitoneal infection in rats. Methods: The intraperitoneal infection rat model was established by cecal ligation and puncture(CLP) . Laser Doppler blood flow meter on micro - circulation was used to examine the gastric mucosal blood flow and the vitreous electrode was used to measure gastric mucosal pH. Results: The gastric mucosal blood flow (GMBF) of infected group was significantly lower than that of controlled group at 3 hours after CLP, and rapidly dropped to the lowest at 12 hours post - perforation. By the time of 48h following CLP, GMBF of infected group still remained lower than that of controlled group. The gastric mucosal blond flow (GMBF) of Danshen group was significantly raised much more than that of infection group. Gastric mucosal pH of infected group decreased significantly compared with the controlled group at 6h after perforation, and rapidly dropped to the lowest at 12h post - perforation. At 24h after perforation, pH of Danshen group was lower than that of the controlled group. Conclusion: The reduce of GMBF might play an important role in gastric mucosal barrier damage following severe abdominal infection induced by CLP, and the application of Danshen Injection in earlier period could prevent the occurrence of stress ulcer.
