摘  要:目的 探讨原发性高血压 (EH)患者伴左心室肥厚 (LVH)和不伴LVH红细胞内游离Ca2 + 浓度、红细胞膜Ca2 + Mg2 + ATPase活性的变化及其相互关系。方法 检测 30名正常血压者 (NT组 ) ,82例轻度和中度EH患者 (其中LVH组 4 0例 ,非LVH组 4 2例 )外周血红细胞内Ca2 + 浓度及红细胞膜Ca2 + Mg2 + ATPase活性。结果  (1)EH组外周血红细胞内Ca2 + 浓度显著高于NT组 ,红细胞膜Ca2 + Mg2 + ATPase活性明显低于NT组。 (2 )EH患者中LVH组外周血红细胞内Ca2 + 浓度高于非LVH组 ,红细胞膜Ca2 + Mg2 + ATPase活性低于非LVH组 ;(3)相关分析表明LVMI与红细胞内Ca2 + 浓度呈正相关 ,与红细胞膜Ca2 + Mg2 + ATPase活性呈负相关。结论 EH患者尤其是伴LVH时外周血红细胞内Ca2 + 浓度升高而红细胞膜Ca2 + Mg2 + ATPase活性降低 ,提示LVH与红细胞内Ca2 + 过度超负荷有关。Objective To investigate the relationship between intracellular free calcium and cell membrane Ca 2+ -Mg 2+ -ATPase activity of erythrocyte in essential hypertension (EH) patients with and without left ventricular hypertrophy (LVH). Methods In thirty normotensives and eighty-two EH patients (forty patients with LVH and forty-two without LVH), the in tracellular free calcium level of erythrocyte was determined by flow cytometry using a new indicator named Flo-3/AM. Cell membrane Ca 2+ -Mg 2+ -ATPase activity of erythrocyte was measured by a colorimetric method. Results The intracellular free calcium level of erythrocyte in EH patients was statistically higher than that in normotensives (1286±96 nmol/L vs 1013±90 nmol/L, P <0.01), with lower membrane Ca 2+ -Mg 2+ -ATPase activity (77.5±15.4 μmol P i/gHb·h vs 115.7±19.1 μmol P i/gHb·h, P <0.01). The LVH patients had significantly higher intracellular free calcium level and lower membrane Ca 2+ -Mg 2+ -ATPase activity compared with the non-LVH group. Conclusion EH patients with LVH have high intracellular free calcium level and lower membrane Ca 2+ -Mg 2+ -ATPase activity of erythrocyte. Intracellular free calcium overload may be one of the mechanism leading to LVH.
