摘 要:目的探讨 2型糖尿病患者的肾小管功能与肾小球功能变化的相关性。方法采用放射免疫法测定 70例 2型糖尿病患者的 2 4 h尿微量白蛋白 (UMA)、尿α1 微球蛋白 (α1 - MG)、β2 微球蛋白 (β2 - MG)及血尿素氮、肌酐等指标变化 ,并设正常对照组 30例。结果 2型糖尿病患者 2 4 h尿微量白蛋白正常组、微量白蛋白尿组及临床白蛋白尿组尿 α1 - MG及尿β2 - MG均显著高于正常对照组 (P<0 .0 5 ,0 .0 1) ,且 2 4 h尿 UMA与尿 α1 - MG及尿 β2 - MG均呈正相关 (r=0 .4 2 7,0 .4 4 6 )(均 P<0 .0 1)。结论尿α1 - MG、尿β2 - MG可作为 2型糖尿病肾病早期诊断指标之一 ,同时也表明在糖尿病肾病早期可能存在肾小管功能的损害 ,提示肾小管损害在糖尿病肾病的发生发展中起着非常重要的作用。Objective To explore the correlation of functional alterations of renal canaliculus and glomerulus in patients with type 2 diabetes mellitus.Method 70 of type 2 diabetes patients and 30 of controls were selected and detected with radioimmunoassay on their levels of 24 hours urine micro-albumin(24 h UMA),α 1 micro-globulin(α 1-MG),β 2 micro-globulin(β 2-MG),and blood urea nitrogen(BUN) and creatinine in serum.Results Levels of urine α 1-MG and β 2-MG were significantly higher in groups of normal 24 h UMA,UMA and clinical albumin urine than in group of controls,respectively(P<0.05,P<0.01).There were positive correlations of 24 h UMA with urine α 1-MG and urine β 2-MG(r=0.427,0.446,P<0.01,respectively).Conclusion Urine α 1-MG and β 2-MG could be items of early diagnosis on type 2 diabetic nephrosis.There existed functional impairments in renal canaliculus on the early stage of diabetic nephrosis.
