摘  要:本文旨在说明电刺激治疗仪在治疗妇女应力性尿失禁、逼尿肌不稳定及混合型尿失禁 ,可获得良好的效果。经由置放在阴道内的电极探头 ,透过主机的微电流能量的调控 ,间歇式地刺激在骨盆底肌内的会阴神经 (pudendalnerve)系统。在逼尿肌不稳定的病患 ,经由反射作用抑制膀胱不自主的收缩。而在真应力尿失禁病患 ,则是使阴部运动纤维之运动神经元 (motorneuron)细胞产生急剧的动作电位 ,加速传导至尿道横纹肌之肌肉组织 ,而增加了尿道闭锁的压力 ,阻止了尿液渗漏和失禁 。The paper explains the clinical application of the electrical muscle stimulator for female urinary incontinence. The electrical muscle stimulator shows its treatment effects in genuine stress incontinence, detrusor instability, urge incontinence and mixed incontinence. An electrode probe is placed in the vagina, and electrical current is delivered to stimulate the pudendal nerve intermittently. In detrusor instability, the pelvic floor stimulation (PFS) depresses or eliminates the uninhibited bladder contractions through the pudendal to pelvic nerve reflex. In genuine stress incontinence, the stimulation activates pelvic muscle contractions through pudendal to pudendal nerve reflex (known as anal wink reflex). The stimulation speeds up the bladder's inhibited action (in detrusor instability) and induces a motion potential of the motor neuron (in genuine stress incontinence). This motion potential transmits to the striated muscles of the urethra and raises the closure pressure of the urethra. The result prevents and treats the urinary incontinence. Such a treatment helps the patients to improve their life quality and even sexual life.
