充血性心力衰竭的改善对老年人肾功能的影响

摘　　要：目的通过观测老年充血性心力衰竭时肾功能的变化及心力衰竭改善后肾功能前后进行比较,了解老年充血性心力衰竭对肾功能的影响。方法32例老年充血性心力衰竭患者,平均年龄75.36±5.63岁,采用国产氨力农,250mg/d,疗程14天,治疗前后观察左室舒张、收缩功能,肾功能变化。结果老年充血性心力衰竭患者的心功能经国产氨力农治疗后心功能明显改善:氨力农能有效地改善心功能,同时肾功能得到改善:血、尿β2微球蛋白降低,尿白蛋白减少(P<0.05),血尿素氮、肌酐减少但统计学无差异(P>0.05)。结论对老年充血性心力衰竭患者,国产氨力农在改善充血性心力衰竭的同时,肾功能也得到改善。血、尿β2微球蛋白、尿白蛋白可作为心力衰竭时观察肾功能的指标。Objective To observe the change of renal function in elderly patients with congestive heart failure before and after treatment with amrinone., to study the effect of congestive heart failure on renal function in elderly patient. Methods 32 elderly cases with congestive heart failure,the average age 75.36±5.63 ,were treated with intravenous injection of amrinone 250mg/d for 14 days. Before and after treatment, parameters such as function of left ventricular,serum and urine β2-MG, urinary albumin excretion, and renal function were measured. Results Before and after treatment of amrinone, MRI showed that cardiac function and renal function had been improved effectively in elderly patients with congestive heart failure. Function of left ventricular diastole and systole had been improved significantly; serum and urine β2-MG,urinary albumin excretion had been declined significantly; BUN and Scr had been decreased, but there was no statistical significance. Conclusions It is shown that Amrinone, using doses of 250mg daily introvenously, can improved cardiac function and renal function effectively. It is probable that the positive effect of amrinone on renal function is attributable to changed cardiac function. Serum and urine β2-MG are the best and the most sensitive parameters of renal function in elderly patients with congestive heart failure.
