体位干预对经尿道前列腺切除术老年患者循环呼吸功能的影响

摘　　要：目的 :探讨体位干预的作用 ,预防或减轻经尿道前列腺切除术 (TURP)对老年患者循环呼吸功能的影响。方法 :选择行TURP且年龄 >6 0岁的老年患者 6 0例 ,随机分为两组 ,对照组术中采用常规的膀胱截石位 ,干预组在手术的不同时段放置不同的体位。监测心率 (HR)、无创血压 (NBP)、心电图 (EKG)、动脉血氧饱和度 (SPO2 )及临床症状。结果 :对照组血压波动明显大于干预组 ,差异有显著性 (P <0 .0 1) ,胸闷、气促、心慌、恶心、呕吐、低SPO2 发生率也高于干预组。结论 :干预体位可以在一定程度上预防或减轻TURP对老年患者循环呼吸功能的影响 ,提高手术的安全性。Objective: To observe effect of posture alteration on circulatory and respiratory system under transurethral prostatectomy. Method :60 elderly patients scheduled for transurethral prostatectomy(TURP )were randomly divided into control group with lithotomy position and experimental group with posture alteration during the operation . Monitoring patients' heart rate(HR), noninvasive blood pressure(NBP) ,electrocardiogram(ECG), pulse oxygen saturation(PaO 2) and clinical symptom .Result: Blood pressure changes were significantly increased ( P <0.01) and occurance of chest discomfort, shortness of breath, palpitaions, as well as vomiting and nausea , were significantly increased in control group comparing with experimental group , PaO 2 in experimental group was also significantly higher than control group. Conclusion: A certain degree posture alteration can prevent or relieve fluctuation on elderly patients' circulatory and respiratory function under TURP.
