摘 要:目的 :探讨Bcl- 2、Bax在自身免疫性甲状腺疾病 (AITD )的表达及意义。方法 :采用免疫组化S -P法检测 54例GD、41例HT及 1 0例正常甲状腺组织作对照 ,用Mias99系统定量分析。结果 :GD、HT的Bcl- 2、Bax表达增高 ,GD的Bcl- 2表达水平高于HT ,Bax表达低于HT(P <0 .0 1 ) ;GD的Bcl- 2阳性颗粒面积及积分光密度与Bax之比高于HT及正常对照 ,HT之比低于正常。结论 :甲状腺Bcl- 2、Bax异常表达 ,尤其是Bcl- 2 /Bax值失衡与AITD的病理过程密切相关。GD的Bcl- 2相对高表达所致的细胞凋亡抑制可能是GD腺体增生、功能亢进的重要因素之一 ;HT的Bax相对高表达所产生的促凋亡效应与HT滤泡破坏。Objective: To investigate the expression of Bcl-2 and Bax in thyrocyte of autoimmune thyroid disease(AITD) and its significance.Methods: 54 cases of Graves'disease(GD),41 cases of Hashimoto's thyroiditis(HT) and 10 normal thyroid specimens were selected.The expression of Bcl-2 and Bax were detected by immunohistochemical S-P method and analyzed by Mias99 pathological image system. Results: The positive granule area, average light density and total light density of Bcl-2 and Bax in both GD and HT thyrocytes were higher than those in normal thyrocytes( P < 0.01 ). The thyrocytes of GD were strongly immunostained to Bcl-2 while the thyrocytes of HT were strongly immunostained to Bax. The ratios of positive granule area and total light density of Bcl-2 to Bax in GD thyrocyte were higher than that in normal thyroid. On the contrary,those in HT thyrocyte were lower than those in normal thyroid .Conclusion : The abnormal expression of Bcl-2 and Bax thyrocyte, especially the lost of Bcl-2 and Bax equilibrium may contribute to the pathogeneses of AITD. The increased Bcl-2 levels in GD thyrocytes should favor thyrocyte survival, hypertrophy and the enhanced expression of Bax in HT thyrocyte may promote thyrocyte apoptosis and follicle disruption.
