煤矿工人心脏储备无创性指标的调查研究

摘　　要：目的　为了获得煤矿工人心脏储备指标的数据。方法　 2 6名煤矿工人和 2 7名教职员纳入该调查研究。不同运动负荷下运动后心率对安静时心率增加的相应倍数定义为心率变化趋势 (HRCT) ;不同运动负荷下运动后第一心音幅值 (S1)对安静时S1幅值增加的相应倍数定义为心力变化趋势 (CCCT) ;完成规定的运动量的 1/4所得CCCT记为CCCT( 1/4 ) ;舒张期 /收缩期时限比值 (D/S)作为心脏舒张期自身供血时限的一个指标。结果　该组煤矿工人和对照组的 (D/S)分别为 1.96± 0 .3和 1.70± 0 .2 2 ,数据间的差异显著 (P <0 .0 1)。S1幅值、心率在运动试验中的变化方向与传统指标心率 -收缩压积在运动试验中的变化方向一致 ,煤矿工人、对照组以及所有受试者的S1幅值运动前后的差异都显著 (P <0 .0 1)。结论　通过体力劳动对心脏的效应 ,延长D/S比值 ,能使心脏舒张期自身供血时限更加充分 ;可以用心肌收缩能力变化趋势来无创地评估体力劳动者的心力储备。Objective To gain the coal miners' data of the heart reserve index . Methods This research contains twenty_six coal miners and twenty_seven teachers and clerks.The definition of the changing tendency of the rate of the heart is the increasing relevant multiple between the rate after the exercises with various load and that in silent state(HRCT);.The definition of the changing tendency of the systolic ability of the myocardium is the increasing relevant multiple between the S1 range value after finishing the exercises with various loads and that in silent state (CCCT); The CCCT after completing one-forth assigned exercises recorded as CCCT(1/4 ); The time limit ratio of the systole and diastole is used as a index which the heart provides blood to itself in the time limit of the diastole.Results The two index in this research separately are:1.96±0.3 and1.70±0.22;the difference is very significant(P<0.01).The changing direction of the S1 range and the rate of the heart in the exercise test are the same comparing with the tradition ways.All the acceptors S1 range are quite different around the exercise (P<0.01).Conclusion Using the heart effect through the manual works,prolong the D/s ratio,in order to well arouse the time limit of diastole to supply blood for itself. And also can use this changing tendency of the heart systole to evaluate the ability systolic of the myocardium of the manual workers.
