大鼠系列减体积原位肝移植的实验研究

摘　　要：目的　探讨不同类型的大鼠减体积性肝移植的存活时间、肝再生及超微结构改变。方法　采用二袖套、胆管端端缝合法建立 70 %、6 0 %、5 0 %、4 0 %和 30 %大鼠减体积性肝移植模型 ,观察术后受体鼠的生存时间、肝重 /体重、移植肝 /受体原肝的变化及肝功能、组织病理和超微结构改变。结果　全肝移植和 70 %、6 0 %、5 0 %、4 0 %和 30 %大鼠减体积性肝移植 (re-duced- size liver transplantation,ROL T)的 1周存活率分别为 95 %、90 %、85 %、70 %、1 5 %、0 ;2周存活率分别为 85 %、80 %、75 %、6 0 %、0、0。 30 %和 4 0 % ROL T的生存曲线与其它组相比均有显著性差异 ;1 0 0 % OL T、70 % ROL T、6 0 % ROL T、5 0 %ROL T、4 0 % ROL T、30 % ROL T的移植肝 /受体体重分别为 3.5 3± 0 .36 %、2 .86± 0 .37%、2 .4 5± 0 .2 5 %、1 .96± 0 .2 6 %、1 .5 9± 0 .1 8、1 .2 2± 0 .2 3;70 % ROL T、6 0 % ROL T和 5 0 % ROL T术后 1周移植肝再生至术前水平 ,而 4 0 % ROL T、30 % ROL T术后早期虽有一定的再生 ,但常死于肝功能栓 ;术后超微结构基本正常 ,但 4 0 % ROL T和 30 % ROL T可见肝窦内皮细胞水肿、肝窦充血 ;光镜可见中央静脉及肝窦扩张。结论　成功的大鼠减体积性肝移植其移植肝 /受体体重?Objective To explore the survival time and liver regeneration in different kind of reduced size liver transplantation in rats.Methods By modified two cuff vascular anastomoses and end to end suture for bile duct,rat different reduced size orthotopic liver transplantation were performed to explore the recipient body weight,graft weight,recipient original liver weight,liver function,histological and electron microscopic findings in comparison with whole rat liver transplantation.Results One week survival rate of the whole liver transplantation,70% reduced size liver transplantation(ROLT),60% ROLT,50% ROLT,40% ROLTand 30% ROLT group was 95%?90%?85%?70%?15%?0,and their ratio of graft weight and recipient body weight was 3.53±0.36%?2.86±0.37%?2.45%±0.25%?1.96±0.26%?1.59±0.18?1.22±0.23 respectively.The grafts regenerate rapidly in size almost equal to the recipient liver over the course of the first week in 70% ROLT and 50% ROLT,but 40% ROLT and 30% ROLT often died of liver failure in the early days after operation.The dilation of the central vein and sinusoids was found in ROLT.Conclusion In successful and long term survival rat with redeced size liver transplantaion,the ratio of graft and recipent weight should over 1.96±0.26%(50%ROLT).The liver regeneration is enhanced after liver transplantation and restored its original size at the first week in most cases,but small for size grafts(40% or 30%)often die of liver failure in early days.
