肝移植时供肝耐受无心跳热缺血的安全时限实验研究

摘　　要：目的　探讨肝移植时供肝耐受无心跳热缺血损伤的安全时限。方法　建立广西巴马小型猪原位肝移植动物模型并按移植前供肝经历心脏停搏时间 0、30、4 5、6 0 min分为 4组 ,观测肝移植后各组一周存活率、肝功能、肝脏病理和肝脏能量代谢以及术中肝脏复流后微循环改变。结果　上述各组术后一周存活率分别为 :1 0 0 % (5 / 5 )、1 0 0 % (5 / 5 )、6 0 % (3/ 5 )、2 0 %(1 / 5 )。肝脏能量代谢等指标的改变在供肝无心跳热缺血时间 30 m in前是可逆的 ,随心跳热缺血时间的延长逐渐向不可逆演变。结论　在本实验条件下 ,巴马小型猪心脏停搏供体肝移植时供肝耐受无心跳热缺血损伤的安全时限约为 30 mObjective To investigate the safe duration limits of tolerance to warm ischemia of liver grafts from non heart beating donors(NHBDs).Methods Guangxi BAMa Miniature pig orthotopic liver transplantation was performed in 4 groups with 0,30,45,60min cardiac arrest time in the donors.1 week survival rate of recipients, liver pathological observation, hepatic function, energy metabolizing of liver, liver microcirculation after reperfusion were compared. Results With the prolonged duration of warm ischemia 1 week survival rate of these 4 groups were 100%(5/5),100%(5/5), 60%(3/5) and 20%(1/5) respectively. Conclusion The safe duration limit of tolerance to warm ischemia of liver grafts from NHBDs is about 30min.
