缬沙坦对高血压左室肥厚患者QT离散度及室性心律失常的影响

摘　　要：目的 :探讨缬沙坦 ( Valsartan,Val)对高血压病左室肥厚 ( LVH)患者左室质量指数 ( L VMI)、心率校正的QT离散度 ( QTcd)及室性心律失常 ( VA)的影响。方法 :高血压病 L VH患者共 5 5例 ,随机分为常规治疗组 ( Rou组 ,n=3 0 ) ,缬沙坦治疗组 ( Val组 ,n=2 5 )。两组分别于治疗前、治疗 1年后用彩色多普勒 Penn法测定 L VMI,分析体表标准 12导联心电图 QTcd并用 Holter记录 VA发生的情况。结果 :LVH患者 L VMI与 QTcd间有显著相关性 ( r=0 .3 6,P<0 .0 1)。 Rou组治疗前后 LVMI为 15 0± 16g· m-2 vs 14 9± 16g· m-2 ( P>0 .0 5 ) ;Val组治疗前、后 LVMI为 15 2± 17g·m-2 vs13 9± 15 g·m-2 ( P<0 .0 1)。Rou组治疗前后 QTcd为 74± 12 ms vs72± 12 ms( P>0 .0 5 ) ,Val组治疗前后 QTcd为 73± 12 ms vs65± 9ms( P<0 .0 5 )。治疗前 Rou组与 Val组间 QTcd无显著性差异 ( P>0 .0 5 ) ,治疗后两组间 QTcd存在显著性差异 ( P<0 .0 5 )。 Rou组及 Val组治疗前 VA恶性程度无显著性差异 ( P>0 .0 5 ) ,治疗后两组间 VA恶性程度比较有显著性差异 ( P<0 .0 5 )。结论 :Val治疗可显著降低 L VMI和QTcd,从而降低AIM: To investigate the effect of valsartan(Val) on left ventricular mass index(LVMI), heart rate corrected QT interval dispersion(QTcd) and ventricular arrhythmia (VA) in patients with left ventricular hypertrophy(LVH) caused by essential hypertension. METHODS: 55 hypertension patients with LVH were divided randomly into two groups. 30 patients in Rou group were treated routinely. 25 patients in Val group were treated with routine drugs plus val. LVMI, QTcd of 12 lead surface ECG and VA malignant degree recorded by Holter were analyzed before and 1 year after the therapy. RESULTS: In the Rou group,the change of QTcd before and after the therapy was not significant (74±12 ms vs 72±12 ms, P >0.05). In the Val group, the change of QTcd was significant before and after the therapy(73±12 ms vs 65±9 ms, P <0.05). Before the therapy QTcd and the malignant degree of VA between the two groups were not significantly different. But both of the two indexes between the two groups were significantly different after the therapy. CONCLUSION: Treatment with Val can significantly reduce the QTcd and the malignant degree of VA in patients with LVH caused by hypertension.
