黄莲素治疗鼻咽癌放化疗后WBC减少症的临床观察

摘　　要：目的　探讨黄莲素在治疗鼻咽癌放化疗所致的WBC减少症中的作用 ,为寻找安全、有效的抗骨髓抑制药物提供科学依据。方法　对 165例放化疗后致WBC减少症的鼻咽癌患者随机分为 2组 ;治疗组 78例应用黄莲素治疗 ,对照组 87例应用清开灵胶囊治疗 ;2组治疗前后分别进行WBC数和生存质量评估 ,观察中医症候等。结果　WBC治疗后 ,治疗组和对照组WBC计数提供的显效率、有效率、总有效率分别为 43 5 9% ( 3 4/78) ,48 72 % ( 3 8/78) ,84 62 % ( 66/78)和 18 3 9% ( 16/87) ,3 3 3 3 % ( 2 9/87) ,49 43 % ( 4 3 /87) ,两组比较 ,差异有非常显著性 (P <0 0 1) ;治疗组、对照组的生存质量改善率分别为 65 3 9% ( 5 1/78)和 41 3 8% ( 3 6/87) ,差异有非常显著性 (P <0 0 1)。治疗组、对照组的中医证候显著改善率、部分改善率和总改善率分别为 5 3 85 % ( 4 2 /78)、 3 2 0 5 % ( 2 5 /78)、84 62 % ( 66/78)和 19 5 4% ( 17/87)、 3 7 93 % ( 3 3 /87)、 5 8 62 % ( 5 1/87) ,差异有非常显著性 (P <0 0 1)。结论　黄莲素在鼻咽癌放化疗后所致的WBC减少症的治疗中是一种很有前景 ,值得深入研究的中药。Objective To study the funtion of berberine treating nasopharyngeal carcinoma patients with leukopenia by radiotherap and chemiotherapy. Methods 165 patients with leukopenia after being treated nasopharyngeal carcinoma by radiotherapy and chemiotherapy were randomized into two groups:therapy group(berberine),78 cases;control group(Qingkailing),87 cases.The nurmber of leucocyte was accounted for both two groups,the quality of life was evaluated and the symptoms was observed by Chinese medical science. Results The marked utility rate,utility rate and omni-utility rate of the Change of leucocyte number were 43.59%(34/78),48.72%(38/78)and 84.62%(66/78)in therapy group and 18.39%(16/78),33.33%(29/87)and 49.43%(43/87)in control group,(P<0.01);the amelioration rate of the quality of life in therapy group was 65.39%(51/78)while in control group was 41.38%(36/87),(P<0.01);the marked amelioration rate,partly amelioration rate and omni-amelioration rate of chinese medical science symptoms were 53.85%(42/78),32.05%(25/78)and 84.62%(66/78)in therapy group and 19.54%(17/87),37.93%(33/87)and 58.62%(51/87)in control group(P<0.01). Conclusion Berberine is a very oleo-prospect traditional Chinese medicine,and it is worthy further study.
