摘 要:目的 :探讨缺血再灌注 (I- R)对血小板 P-选择素 (Ps)、白细胞 CD11b及心肌组织单核细胞趋化蛋白 - 1(MCP- 1)基因表达的影响及硫酸镁对 I- R损伤的保护作用。方法 :建立在体大鼠 I- R模型 ,然后把大鼠随机分成 3组 :假手术组 (sham组 )、缺血再灌注组 (I- R组 )、硫酸镁组 (Mg2 + 组 )。用流式细胞仪及逆转录聚合酶联反应 (RT-PCR)分别测定 I- R组大鼠术前、再灌注 (R) 5、30、6 0、180、36 0 min血小板 Ps和白细胞 CD11b表达及心肌组织MCP- 1m RNA表达。结果 :I- R组血小板 Ps表达在 R5开始上升 ,在 R6 0达高峰 (P<0 .0 1) ;白细胞 CD11b表达显著增高 (P<0 .0 1及 P<0 .0 5 ) ;心肌组织 MCP- 1m RNA在 R6 0开始上升 ,至 R36 0达高峰 (P<0 .0 1) ;血清 Ca2 + 浓度均显著增高 (P<0 .0 1及 P<0 .0 5 )。Mg2 + 组血小板 Ps、MCP- 1m RNA的表达及血清 Ca2 + 浓度均降低 ,但对白细胞 CD11b的表达无影响。结论 :I- R促进大鼠细胞黏附分子、趋化因子的表达 ;硫酸镁可能通过抑制血小板 Ps及心肌组织 MCP- 1m RNA的表达 ,降低血清 Ca2 + 浓度 ,从而起到心脏保护作用。Objective: To investigate the effect of ischemic reperfusion(I-R) on the expression of P-selectin(Ps) of platelets,CD11b of leukocytes,monocyte chemotactic protein-1(MCP-1) mRNA of myocardial tissue and the protective role of MgSO 4.Methods:Ischemic reperfusion rat model was established in the experiments.All animals were randomly divided into 3 groups:sham group,I-R group and Mg 2+ group.The expression of Ps of platelets and CD11b of leukocytes was determined by flowcytometry and the expression of MCP-1 mRNA of myocardial tissue was measured by RT-PCR admission and at 5,30,60,180,360 min of reperfusion (R5,R30,R60,R180,R360). Results:The expression of Ps of platelets in I-R group began to rise at R5 and peaked at R60(P<0.01);the expression of CD11b of leukocytes was significantly higher than that of the control groups(P<0.01 and P< 0.05);the expression of MCP-1 mRNA of myocardial tissue began to rise at R60,reaching the highest at R360(P< 0.01);the concentration of Ca 2+ of the serum was significantly higher than that of the control groups(P< 0.01 and P<0.05).MgSO 4 inhibited the expression of Ps of platelets and MCP-1 mRNA of myocardial tissue and decreased serum Ca 2+ concentration,but did not affect the expression of CD11b of leukocytes.Conclusion:I-R can increase the expression of cell adhesion molecules and cell chemotactic factor.MgSO 4 may protect myocardial tissue from ischemic reperfusion injury by inhibiting the expression of Ps of platelets and MCP-1 mRNA of myocardial tissue and decreasing the concentration of Ca 2+ of the serum.
