摘  要:目的 :探讨 2型糖尿病 (DM)患者尿TGF - β1、Ⅳ胶原 (Ⅳ -C)含量的变化及其临床意义。方法 :6 1例 2型DM患者根据尿白蛋白排泄率 (UAER)分为正常白蛋白尿组 (Ⅰ )、微量白蛋白尿组 (Ⅱ )和大量白蛋白尿组 (Ⅲ ) ,分别检测各组患者尿TGF - β1和Ⅳ -C的含量 ,并与 30例正常对照组 ((NC)进行比较。结果 :糖尿病各组患者尿TGF - β1和Ⅳ -C含量显著高于正常对照组 (p <0 0 5 ) ,且随着UAER增加其水平亦呈逐年递增趋势 ,组间比较差异显著 (p <0 0 1)。相关分析表明 ,尿TGF - β1与尿Ⅳ -C、UAER及尿α1-m之间呈显著正相关 (p <0 0 1)。结论 :TGF - β1和Ⅳ -C在糖尿病肾病 (DN)的发生发展中起着重要作用。测定DM患者尿TGF- β1和Ⅳ -C的含量 ,不仅可作为诊断DN早期肾损害较敏感的指标 ,还将有助于监测。Objective To investigate the urinary excretion rates of TGF-β 1 and Ⅳ-C in diabetic patients (DM 2) and their clinical significance. Methods Sixty-one cases of (DM 2) patients were divided into three groups acccording to urinary albumin excretion rate (UAER): normoalbuminuric group(Ⅰ) (<20μg/min,n=25),microalbminuric group(Ⅱ)(20~200μg/min,n=19),macroalbuminuric group(Ⅲ)(>200μg/min,n=17). The urinary excretion rates of TGF-β 1 and Ⅳ-C were determined in all the cases and 30 controls. Results The excretion rates of TGF-β 1 and Ⅳ-C in various DM groups were markedly higher than those in controls ( p <0 05). Moreover, the excretion rates of TGF-β 1 and Ⅳ-C increased with the increase of UAER and there were significant differences among the three groups ( p <0 01). Urinary TGF-β 1 was also positively correlated with Ⅳ-C UAER and α 1-m ( p <0 01). Conclusion This study suggested that TGF-β 1 and Ⅳ-C might play a very important role in the development of nephropathy in DM 2. Measurements of urine TGF-β 1 and Ⅳ-C could be used for early diagnosis of diabetic renal dysfunction as well as monitoring the disease process.
