摘 要:目的 了解我院洋葱伯克霍尔德菌医院感染情况。方法 对“军字一号”医院感染监控系统提供的 2 0 0 0年1月~ 2 0 0 2年 11月医院感染病例 6 0 3份 ,洋葱伯克霍尔德菌医院感染 2 3例进行调查。结果 2 3例感染患者 ,占同期医院感染病例的 3.81% ,死亡 6例 ;基础疾病以烧伤、恶性肿瘤、多发骨折占多数 ;感染发生部位以呼吸道、伤口和血液为多 ;洋葱伯克霍尔德菌对多种抗菌药物耐药 ,对阿莫西林 /克拉维酸、妥布霉素、氨苄西林、庆大霉素的耐药率在 90 %~ 10 0 %。结论 洋葱伯克霍尔德菌医院感染多由外源感染所致 ,加强医用水、器械、管路等的消毒灭菌 ,介入检查治疗患者的无菌操作和日常护理 ,可预防洋葱伯克霍尔德菌医院感染 ;已有感染迹象患者的经常性微生物学检查 ,对发现洋葱伯克霍尔德菌医院感染及指导抗菌药物的使用。OBJECTIVE To investigate the clinical characteristics of hospital acquired infection by Burkholderia cepacia. METHODS A total of 23 cases with nosocominal infection (NI) by B. cepacia were retrospectively studied in Jun Zi No. 1 hospital infection control system from Jan 2000 to Dec 2002. RESULTS The incidence rate of the NI by B. cepacia was 3.81% and 6 cases died. The common primary diseases were burns, malignant carcinoma and multifractures. The common infective sites were the lower respiratory tract, wounds and blood. The drug sensitivity tests in vitro showed B. cepacia were multiresistant to commonly used antibiotics. Especially the resistant rate was 90 100% to drugs including amoxicillin/clavulanic acid, tobramycin, ampicillin and gentamicin. CONCLUSIONS The majority of NI by B. cepacia mostly was exogenous infection. The main prevention measures for the NI by B. cepacia should attach importance to the disinfection of water, instrument as well as pipeline, and strict aseptic manipulation in patients with interventional treatment. The frequent tests for microbiology are helpful to find the NI by B. cepacia and to guide the use of antibiotics.
