北京降压0号治疗轻中度原发性高血压的长期疗效观察

摘　　要：目的　评价长期服用北京降压 0号片剂治疗轻中度原发性高血压患者的降压疗效和不良反应。方法　采用多中心长期开放研究设计。舒张压 95～ 114mmHg(1mmHg =0 133kPa)且收缩压 <180mmHg的患者入选本研究 ,采用北京降压 0号每日 1～ 2片治疗 1年 ,观察降压疗效及不良反应。结果　共入选 182例患者 ,其中 175例完成 1年试验。平均服用北京降压 0号剂量为1 2 3～ 1 2 6片 /d ,个体最大使用剂量为 2片 /d。与治疗前比较 ,治疗 2周、8周、6个月和 12个月时收缩压分别下降 11 6 0、2 2 77、2 4 19和 2 6 5 7mmHg ,舒张压分别下降 14 12、17 6 4、18 77和 19 0 7mmHg(P值均 <0 0 0 1)。治疗 8周、6个月和 12个月的总有效率分别为 92 3%、93 3%和 91 4 %。达到正常血压值 (<14 0 /90mmHg)的例数分别为 133例 (73 1% )、14 5例 (81 5 % )和 14 8例(84 6 % ) ,达到理想血压值 (<130 /85mmHg)的例数分别为 6 8例 (37 4 % )、77例 (43 3% )和 92例(5 2 6 % )。治疗 8周时血清肌酐升高者占 7 84 % (P =0 0 0 18) ,但随后又降低至治疗前水平。未发现低钾血症和抑郁倾向。治疗 1年期间曾出现不良反应的人数为 2 4例 (13 4 % ) ,主要为轻度头晕、头痛 ,继续治疗均可自行缓解。无严重不良反应发生。Objective To assess the antihypertensive effect and safety of Beijing Hypotensive No.0 in treatment of essential hypertension for one year. Methods A multi-central open-labeled study was conducted. One hundred and eight-two patients with diastolic blood pressure 95-114 mm Hg and systolic blood pressure <180 mm Hg were treated with Beijing Hypotensive No.0 1-2 tablets per day for one year. The antihypertensive effect and adverse events were observed. Results One hundred and seventy-five patients completed the study. The average dosage was 1.23-1.26 tablets per day, the maximum individual dosage was 2 tablets per day. After 2 weeks,8 weeks, 6 months, 12 months′ treatment,the systolic blood pressure decreased 11.6 mm Hg, 22.77 mm Hg, 24.19 mm Hg and 26.57 mm Hg respectively, diastolic blood pressure decreased 14.12 mm Hg, 17.64 mm Hg, 18.77 mm Hg and 19.07 mm Hg respectively ( P <0.001 comparing with the baseline ). After 8 weeks, 6 months and 12 months′ treatment, the total effective rate was 92.3%,93.3% and 91.4% respectively. Blood pressure controlled in normal range (<140/90 mm Hg) were 133(73.1%),145(81.5%) and 148(84.6%)cases. Blood pressure controlled within 130/85 mm Hg were 68(37.4%), 77(43.3%) and 92(52.6%) cases. The serum creatinine increased 7.84%( P = 0.001 8) in 8 weeks, but it returned to baseline afterward. No hypokalemia and depression occurred. Side effects occurred in 24/182(13.4%) cases during one year′s treatment, most were mild dizziness and headache. No severe side effect occurred. Conclusion Beijing Hypotensive No.0 1 tablet per day can remarkably and safely reduce blood pressure in one-year study. The blood pressure can be controlled in normal range in most patients.
