摘　　要：目的 :研究氯沙坦对心力衰竭 (CHF)病人血肿瘤坏死因子 (TNF α)、内皮素 (ET 1)、醛固酮(Ald)水平的影响。方法 :采用放免法测定CHF病人 (5 4例 )及健康志愿者 (2 8例 )血TNF α ,Ald及ET 1水平。 5 4例CHF病人分为 2组 ,非氯沙坦组给予常规抗心力衰竭治疗 ;氯沙坦组在非氯沙坦组药物治疗基础上加氯沙坦 2 5～ 5 0mg·d- 1,共 1mo。结果 :CHF病人TNF α ,Ald及ET 1水平分别为(2 .7±s 0 .8) μg·L- 1,(195± 33)ng·L- 1,(90±16)ng·L- 1,显著高于健康对照组 [(1.2± 0 .2 ) μg·L- 1,(117± 38)ng·L- 1,(5 9± 11)ng·L- 1,P <0 .0 1];氯沙坦组经治疗后TNF ,Ald及ET 1水平显著降低 ;非氯沙坦组治疗后血Ald水平明显降低(P <0 .0 1)。结论 :氯沙坦可明显降低心力衰竭病人血TNF α及ETAIM:To explore the effects of losartan on aldosterone (Ald), endothelin-1 (ET-1), and tumor necrosis factor (TNF-α) in patients with heart failure (HF). METHODS: The levels of TNF-α, Ald, and ET-1 of patients with heart failure (n=54) and healthy people (n=28) were measured by radioimmunoassay. Fifty-four patients were randomly divided into losartan group and non-losartan group. The losartan group received losartan 25-50 mg,po, qd, for 1 mo in addition to routine therapy (losartan group, n=28) and the non-losartan group received routine therapy only (non-losartan group, n=26) for 1 mo. RESULTS: The levels of TNF-α, Ald and ET-1 of patients with HF were (2.7±s 0.8) μg·L -1,(195±33) ng·L -1,(90±16) ng·L -1,respectively, markedly higher than those of healthy (1.2±0.2) μg·L -1,(117±38) ng·L -1,(39±11) ng·L -1(P<0.05 or P<0.01). The levels of TNF-α, Ald, and ET-1 of patients treated with losartan decreased significantly (P<0.01), and only the level of Ald of patients treated with routine therapy (in non-losartan group) reduced obviously (P< 0.01). CONCLUSION:It is showed that losartan can obviously decrease the levels of TNF-α and ET-1 in patients with HF.
