小儿贲门失弛缓症的手术治疗

摘　　要：①目的　探讨小儿贲门失弛缓症的手术方法和治疗效果。②方法　回顾性分析我院近 1 0年收治的小儿贲门失弛缓症 32例。全部行经腹改良Heller手术治疗 ,2 3例附加Nissen手术治疗。③结果　术后随访 1～ 2年无复发者 ,生长发育正常。单纯行Heller手术者有 2例在术后 1个月左右出现轻度泛酸、嗳气等症状 ;2 3例附加Nissen手术者无胃食管反流症状。④结论　经腹改良Heller手术附加Nissen手术 ,操作简便 ,创伤相对较小 ,可大大减少胃食管反流的发生 。Objective\ To evaluate the operative procedures and outcome for cardiac achalasia in children.\ Methods\ A retrospective study was done for 32 children with cardiac achalasia treated during the past ten years. All of them were operated on by modified Heller procedure and 23 of them combined with Nissen procedure. \ Results\ Followed up 1 to 2 years, no recurrence was found and all patients showed normal growth and development. 2 cases treated by Heller procedure alone had mild gastric acid reflux and burping about 1 month postoperatively, while 23 cases treated by Heller combined with Nissen procedure had no symptom of gastroesophageal reflux.\ Conclusion\ Modified Heller procedure combined with Nissen procedure is easy to perform and less damage. It is an ideal operation for cardiac achalasia in children.
