肿瘤术后游离骨移植及并发症临床分析

摘　　要：目的 :下颌骨肿瘤术后所致缺损通过游离骨移植 ,重建下颌骨的连续性 ,恢复下颌骨功能。方法 :下颌骨造釉细胞瘤切除术后 ,用自体游离髂骨、肋骨及病变下颌骨切除后煮沸再植于下颌骨缺损。结果 :5 1例植骨病人疗效满意 4 9例 ,植骨失败 2例 ,髂骨供骨区出现并发症积液 3例。结论 :自体游离骨移植效果满意 ,仍需要注意控制感染及预防并发症发生 ,自体瘤骨立即再植可避免排异现象 ,并且可以减少手术创伤。Objective:To evaluate the effect in 51 cases of free bone grafting for the immediate repair of mandibular defects.Methods:There were 38 cases repaired by free ilium flap, 10 cases by rib and 3 cases by prompt autotransplantation of boliled tumorous mandible.Results:The operation were successful in 49 patients, but failed in 2 cases. Donor sites complications were found in 3 cases.Conclusions:The effect of free bone grafting can satisfy the need of functional and esthetic reconstruction of mandibular defect. The result also shows that it is acceptable to treat mandibular tumor with prompt autotransplantation of boliled tumorous mandible.
