正常成年人甲状腺体积影响因素分析

摘　　要：目的 :探讨正常成年人甲状腺体积 (TV)的影响因素。方法 :对低碘、碘充足和高碘 3个不同碘摄入量人群进行问卷调查和甲状腺功能、甲状腺自身抗体及甲状腺B超检查 ,筛查出正常成年人群 ,进行TV与年龄、性别、身高、体重、碘摄入量、血清促甲状腺激素 (TSH)水平关系的研究。结果 :3地区成年人TV的大小与年龄、绝经、吸烟及饮酒无明显关系 ;TV随人群碘摄入量减少而增大 ;TV与身高、体重及体表面积呈正相关 (P <0 .0 1) ,与TSH呈负相关 (P <0 .0 1) ;男性TV均明显大于女性 (P <0 .0 5 ) ;但男女人群平均TV/身高及TV/体重比值无明显差异。结论 :尿碘中位数 (MUI)为 10 0～ 6 0 0 μg/L的碘摄入量范围的正常成年人TV随身高、体重 ,特别是体表面积的增加而增大 ,而随人群碘摄入量增加及血清TSH水平升高而缩小 ;TV的大小与年龄、性别、绝经、吸烟。Objective: Our aim was to analyze influence factors of thyroid volum (TV) in healthy adults. Methods: Questionnaire, thyroid function, thyroid autoantibody assay and thyroid B-ultrasound scan were performed in population in 3 areas with different iodine intake. After screening the healthy adults, we investigated the relationship of TV and age, sex, body height, body weight, iodine intake, serum thyroid-stimulating hormone (TSH) level. Results: Adult TV had no relationship with age, sex, menopause, smoking or drinking. With the decrease of iodine intake, TV increased. TV had positive correlation with body height, body weight and body surface area(P<0.01), and negative correlation with serum TSH level(P<0.01). The TV in men was higher than that in women(P<0.05). But the ratio of TV to body weight between sexes showed no obvious difference. Conclusion: In adults whose median urinary iodine (MUI) are between 100 to 600 μg/L, TV increases with the increasing of body height, body weight, especially body surface area, decreases with the increasing of iodine intake and serum TSH level, and has no relationship with age, sex, menopause, smoking and drinking.
