窒息新生儿早期干预及发育障碍保健康复的效益评价

摘　　要：【目的】　评价窒息新生儿早期干预及发育障碍保健康复治疗效果的社会经济效益。　【方法】　对窒息早期综合干预组 44例、康复治疗组 2 3例和窒息对照组 2 8例进行两个效益的评价。评价指标 :四项体格指标 (W /A ,H/A ,W /H ,B/A)和DQ正常者的比例 ,开始保健康复治疗的平均年龄 ,保健康复治疗次数 ,服务项目数 ,费用 ,人均研究成本和家长出勤日数等 12项指标。按健康优先、成本—效果和社会承受能力的原则 ,采用Topsis法对三组的 12项指标进行评价。　【结果】　窒息新生儿发育障碍早期干预康复治疗有明显的社会经济效益 ,其中干预组远期效益最好 (Ci=0 .5 40 6) ,对照组远期效益较差 (Ci=0 .5 2 44 ) ,康复组有社会经济效益 (Ci=0 .3 5 79) ,但比前两者差。　【结论】　窒息新生儿的早期综合性保健干预是投入少 ,且有长远社会经济效益。To evaluate the social and economic benefit of the treating effect of early intervention on asphyxiated neonate and convalescence by health care for the developmental dysfunction. Two benefits were evaluated for asphyxia in 44 cases with early comprehensive interfered group, 23 cases with convalescent treating group and 28 cases with control group. 12 items were taken as evaluating indexes included 4 physical indexes(W/A, H/A, W/H, B/A) and normal ratio of DQ, the average age of beginning health care convalescence treatment, times of health care convalescence treatment, number of service item, the cost, the total cost of per capita study, and the attendance days of the parents, etc. Topsis method was taken according to the principle of health first, cost effect and supportable ability of society. Early interfered treatment with health care convalescence had got an obvious social and economic benefits for the asphyxiated neonates with developmental dysfunction. Long term benefits of interfered group was best one (C i=0.540 6) in 3 groups. Control group (C i=0.524 4) was not better than interfered group. Convalescent group had social and economic benefits(C i=0.357 9),but it was not better than others. [Conclusions] The early comprehensive health care interference to asphyxiated neonate is a less investment, and a sustained developing public hygienic service project which has a long term social and economic benefits for child health care.
