摘 要:目的:探讨肿瘤坏死因子α(tumor necrosis factorα,TNFα)水平与2型糖尿病患者胰岛分泌功能的关系。方法:应用放射免疫法测定95例2型糖尿病患者的血浆TNFα水平,及其相应的代谢生化指标以了解胰岛分泌功能,并与43名正常人作比较。结果:糖尿病患者血浆TNFα水平犤(1.15±0.34)μg/L犦显著高于正常人犤(1.03±0.21)μg/L犦,P<0.05。胰岛分泌功能良好的2型糖尿病患者TNFα水平犤(1.06±0.24)μg/L犦显著低于胰岛分泌功能不良者犤(1.23±0.40)μg/L犦,P<0.01。多元相关与回归分析提示血浆TNFα水平与糖尿病患者的C肽水平呈负相关。结论:2型糖尿病患者TNFα水平与胰岛分泌功能明显相关。Objective:To study the relationship between plasma tumor necrosis factor α(TNF α) and pancreatic function in patients with type 2 diabetes. Methods:Plasma TNF αand biochemistry markers of metabolism in 95 patients with type 2 diabetes were measured by RIA and were compared with 43 normal controls(NC). Pancreatic function was evaluated by measuring plasma peptide C level. Results:The plasma TNF αlevel in patients with diabetes was higher than that in NC[(1.15±0.34) μg/L vs (1.03±0.21) μg/L,P<0.05]. Patients with better pancreatic function had lower plasma TNF αlevel than those with poor pancreatic function[(1.06±0.24) μg/L vs (1.23±0.40) μg/L,P<0.01]. Patients with better pancreatic function had higher body measure index,percentage of fat and fasting plasma insulin but lower insulin assessment index than those with poor pancreatic function. Multiple lineal regression analysis showed that the plasma peptide C level,systolic blood pressure and presence of hypertension were the main factors that had substantial effects on plasma TNF αlevel in patients with diabetes. Conclusion:There is significant relationship between the plasma TNF αlevel and pancreatic function in patients with type 2 diabetes.
