摘  要:为探讨新生儿期血清瘦素、胰岛素样生长因子_I(IGF_I)和胰岛素水平变化及其对新生儿生长发育的影响,采用放射免疫法检测46例健康新生儿静脉血和脐静脉血瘦素、IGF_I和胰岛素水平 ,采用新生儿体重、身长和Rohrers指数[(RI)=出生体重(g)×100/身长(cm)3]估测新生儿生长营养状态。结果显示早期新生儿组血清瘦素水平(0.59±0.43μg/L)明显低于初生组(8.29±6.76μg/L)和晚期新生儿组(1.68±0.58μg/L) ;早期至晚期新生儿组血清瘦素水平的增长值与此期间血清IGF_I、胰岛素和新生儿体重的增长值呈显著正相关(r=0.383,r=0.284,r=0.365) ;初生至早期新生儿组血清瘦素水平的下降值与胰岛素水平、新生儿体重的变化无相关性。提示瘦素在调节新生儿期体重和能量平衡方面起重要作用 ;瘦素可能通过IGF_I。To investigate the dynamic changes of serum leptin,insulin_like growth factor(IGF_I) and insulin levels in newborns and to explore their relationship with neonatal growth and development,the serum levels of leptin,IGF_I and insulin in 46 healthy neonates were measured with radioimmunoassay(RIA)or radioimmunometric assay.The results showed that the mean serum leptin level at 4~6 days of newborns was 0.59±0.43 μg/L,significantly lower than that at birth (8.29±6.76 μg/L),and was also signiflcantly lower than that at 25~28 days of life(1.68±0.58 μg/L).The increase of serum leptin levels significantly correlated with the body weight gain as well as with the increase of serum IGF_I and insulin levels occurred during 4~6 and 25~28 days(r=0.365,r=0.383,r=0.284,respectively).The decrease of serum leptin levels did not correlate with the body weight loss,nor with the reduced levels of serum insulin occcurred at birth or at 25~28 days of life,suggesting that the dynamic changes of leptin levels in neonate had some relevance with their body weights and played an important role in regulation of their body weight and the balance of energetic metabolism in neonate and might collaborate with IGF_I and insulin,to modulate the neonatal growth and development.
