摘 要:目的通过分析2型糖尿病患者尿Smad1水平,探讨Smad1作为生物标志物诊断早期2型糖尿病肾病及评价肾损伤程度的可行性。方法 120例2型糖尿病患者及40名健康志愿者参与试验。糖尿病患者按照尿白蛋白与尿肌酐的比值(ACR)分成四组:正常白蛋白尿组(NAU)、低白蛋白尿组(LAU)、高白蛋白尿组(HAU)和极高白蛋白尿组(VHAU)。其中,LAU组、HAU组和VHAU组被认为是糖尿病肾病组(DN组),NAU组被认为是非糖尿病肾病组(非DN组),健康志愿者被作为对照组。用酶联免疫吸附法测尿Smad1的水平,尿Smad1与尿肌酐之比(SCR)作为标准参数。结果与非DN组比,DN组SCR明显升高(P<0.05),而非DN组与对照组相比,SCR无明显区别(P>0.05),SCR在VHAU组较LAU组及HAU组明显升高,而且HAU组SCR较LAU组明显升高(P<0.05)。Pearson相关分析示SCR与ACR及糖尿病病程明显正相关。结论 Smad1或许可作为DN的潜在诊断参数,可作为DN损害程度的评价指标,而且ACR及糖尿病病程是尿Smad1水平的独立影响因素。Objective Through analyzing urinary Smad1 level in patients with type 2 diabetes,to explore the possibility of Smad1 being a biomarker for early diagnosis and prognosing severity of diabetic nephropathy.Methods The 120 subjects with diabetes and 40 healthy volunteers were enrolled in this study. The former was grouped acording to urine alumin to creatinine ratio(ACR) into four groups: normal albumin in urine(NAU),low albumin in urine(LAU),high lbumin in urine(HAU),and very high lbumin in urine(VHAU). Among those,LAU,HAU and VHAU were regarded as the diabetic nephropathy group(DN group),NAU were regarded as non diabetic nephropathy group(non-DN group),and the healthy volunteers were regarded the control group. Smad1 levels were dete-rmined with ELISA,Smad1 to creatinine ratio(SCR) was used as the standard reference. Results Compared with non-DN group, SCR of DN group was higher(P<0.05),while there was no difference between the non-DN group and control group(P>0.05). The SCR was higher in VHAU group than those in HAU and LAU groups, and higher in HAU than LAU group(P<0.05). Pearson correlation analysis showed that SCR were positively correlatied to A-CR disease-course. Conclusion Smad1 may be as a potential diagnosis parameter for DN and may be used to estimate the severity of DN,furthermore,ACR and disease-course may be as independent impact factors for urinary Smad1 level.
