摘 要:目的观察高氧液对心肌缺血/再灌注心肌梗死面积、心肌结构和细胞凋亡参数的影响,探讨高氧液抗心肌缺血/再灌注损伤的保护作用机制。方法家兔心脏左冠状动脉前降支缺血/再灌注动物模型,缺血30min,再灌注2h。60只家兔随机数字表法分成3组:对照组(I组,n=20)只暴露左冠状动脉前降支,不结扎;缺血/再灌注组(Ⅱ组,I/R组,n=20),在缺血再灌注前10min静脉注射生理盐水(20ml/kg);高氧液治疗组(Ⅲ组,n=20),在缺血前10min静脉注射高氧液(20ml/kg)。分别按TCC法染色,用图像分析系统计算梗死面积;原位末端TUNEL法检测细胞凋亡指数;免疫组化ABC法检测Fas、Bcl-2的含量,同时电镜观察心肌细胞的超微结构。结果III组心肌梗死细胞面积为(2.29±0.02)cm,与II组的(3.38±0.07)cm比较,梗死面积明显缩小。II组的AI(20.8±0.36),明显高于I组(4.1±0.25),P<0.01,t=170.4,III组的AI(13.3±0.35)显著低于II组,P<0.01,t=66.8,但仍高于I组,P<0.01,t=92.8。I组Fas的OD值为2.80±0.07,II组3.70±0.05,III组3.10±0.04。I组Bcl-2的OD值为2.7±0.02,II组0.60±0.03,III组2.90±0.02。结论高氧液具有抗心肌缺血/再灌注损伤的作用,其作用机制可能是通过调节Fas和Bcl-2介导的心肌缺血/再灌注细胞凋亡而实现。Aim To observe the effects of intravenous hyperoxia liquid on the myocardial infarct size,myocardial ultrastructure,myocardial cell apoptosis and investigate the protective mechanism of hyperoxia liquid against ishemia/reperfusion(I/R) injury.Methods Sixty SD rabbits were anesthetized with abdominal sodium pentobarbital,tracheotomized and mechanically ventilated.After 30 min ischemia and 2 hour reperfusion,the rabbits were randomly divided into 3 groups:control group(n=20) in which left anterior descending coronary artery was exposed and a piece of silk thread was placed around the artery but not tied;I/R group(n=20) in which normal saline (20 ml/kg) was injected into vein before I/R; hyperxia liquid group(n=20) which received intravenous hyperoxia liquid(20 ml/kg) at 10min before I/R.The animals were then sacrificed and heart was harvested for determination of myocardial infarct size(by TTC) and ultrastructure examination(electron microscope).Apoptosis was identified by TUNEL and apoptosis index(AI) was obtained.The expression of Fas,Bcl 2 protein was measured by immunohistochemical ABC technique.Results The infarct size of hyperxia liquid group was ( 2.29± 0.02) cm,significantly smaller than that of the I/R group [( 3.38± 0.07) cm].The apoptosis index was 20.8± 0.36 in the I/R group,significantly higher than in the control (4.1± 0.25) and hyperxia liquid( 13.3± 0.35) groups respectively(P< 0.01,t=170.4;P< 0.01,t=66.8).The apoptosis of the hyperxia liquid group was significantly higher than that of the control group (P< 0.01,t=92.8) .The OD score of Fas protein was 2.80± 0.07,3.70± 0.05, and 3.10± 0.04 in the control,I/R and hyperxia liquid groups respectively. The OD score of Bcl 2 protein was 2.7± 0.02, 0.60± 0.03 and 2.90± 0.02 in the three groups respectively.Conclusions Hyperoxia liquid protects myocardium from I/R injury by modulating the expression of Fas and Bcl 2 protein and inhibits apoptosis following myocardial I/R.<P>
