透明质酸钠治疗老年类风湿关节炎3年随访研究(英文)

摘　　要：目的观察透明质酸钠(HA)关节腔注射治疗老年类风湿关节炎(RA)临床疗效。方法1999-10/2003-03期间风湿科就诊的31例老年RA患者共41个对全身治疗抵抗的关节(膝36个,肩5个)接受关节腔注射HA治疗。每周1次,持续5周。记录治疗前后关节静息痛、活动痛、被动活动痛、肿胀、浮髌试验、局部皮温、步行距离痛和晨僵时间。结果完成治疗后,老年RA患者关节活动痛、静息痛、关节肿胀、浮髌试验、局部皮温、步行距离痛及晨僵时间均较治疗前明显改善(t=2.54-4.23,P<0.01)。被动活动痛评分有下降趋势(t=2.12,P=0.125)。结论全身治疗抵抗的老年RA患者部分关节腔注射HA有效。Aim To investigate the clinical efficacy of sodium hyaluronate (HA) given by intra articular injection for elderly onset rheumatoid arthritis (RA). Methods 31 patients with elderly onset RA (36 knees, 5 shoulders) received 20 mg HA by intra articular injection for 5 consecutive weeks. Clinical parameters were monitored before and after injection, including pain (pain at rest, pain in motion and pain in passive motion), inflammation (swelling, patellar ballotement and local warmth), pain on walking and morning stiffness time. The duration of assessment was from 2 months to 3 years in 29 patients after the treatment. Results Pain at rest, pain in motion, swelling, patellar ballottement, local warmth, pain on walking and morning stiffness time were improved significantly after the treatment (t=2.54- 4.23,P< 0.01). There was no significant difference in pain in passive motion between them. Conclusion HA is an effective and safe drug for elderly onset RA patients given by intra articular injection.
