摘  要:目的 通过对挫伤脑组织中神经细胞凋亡现象及凋亡相关基因Bcl- 2 /Bax的蛋白表达和caspase - 3表达的研究 ,探讨创伤后脑组织中是否存在凋亡以及凋亡在脑外伤病理过程中所起的作用 ,凋亡相关基因Bcl - 2 /Bax和caspase - 3表达的变化。方法 采用流式细胞仪检测凋亡 ,采用免疫组化法检测Bcl- 2 /Bax及caspase- 3的表达。结果 脑组织中细胞凋亡与患者的病情严重程度 (t =3 77,P =0 0 0 1)及受伤时间 (t =2 34,P =0 0 0 7)有关 ,而与患者预后无关 (χ2 =0 6 9,P=0 5 0 )。Bcl- 2阳性表达与预后有关 (χ2 =5 16 1,P =0 0 2 3) ,caspse- 3及Bcl- 2阳性表达与患者性别、年龄、GCS评分、受伤时间无关 (P >0 0 5 )。caspase - 3阳性与阴性表达间凋亡百分数差异显著 (t =0 6 2 1,P =0 0 2 4 ) ,而Bcl- 2阳性与阴性表达凋亡百分数差异不显著 (t =0 5 84 ,P =0 5 6 4 )。所有患者Bax均表达阳性。结论 创伤后脑组织中存在凋亡 ,其在创伤脑组织的病理过程中起一定作用 ,凋亡程度与患者病情严重程度及受伤时间有关 ,Bcl- 2的蛋白表达与预后相关 ,而caspase - 3的阳性表达可直接反映组织凋亡程度。Objective To study on whether the apoptosis existed and the change of gene expression related to apoptosis by investigation of apoptosis of the cerebral contusion and expression of Bcl-2/Bax and caspase-3 related to apoptosis in human.Method We researched that apoptosis and gene expression of Bcl-2/Bax and caspase-3in brain tissues by FCM and immunohistochemical.Results Analysis showed that apoptosis of neuro-cell was related to patient's injury serious level and injury time(P<0 05),but was not related to prognosis.The positive expression of Bcl-2 was related to prognosis(P<0 05),but was not related to sex?age?GCS and injury time.The caspase-3 was highly correlated with the apoptosis,which showed the expression of caspase-3 was an indicator of apoptosis degree.Conclusions Apoptosis occurred in human brain after actue injury which fulfilled a function in pathological transformation of brain tissues. The expression of Bcl-2 was related to prognosis. The expression of caspase-3 could reflect the levels of apoptosis.
