病灶清除椎间植骨治疗胸腰椎结核15例报告

摘　　要：目的　观察病灶清除椎间植骨治疗胸腰椎结核的疗效。方法　全组 15例 ,病灶涉及 2节椎体10例 ,3节椎体 5例。术前后凸畸形成角 13°～ 4 5°,平均 30°。手术采用病灶清除椎体间植骨融合 ,使用自体肋骨 5例 ,髂骨 10例。植骨加前路钢板固定 2例。结果　平均随访 4年 ,优良率 92 % ,植骨融合率 97%。术后后凸畸形成角 5°～ 2 0° ,平均 14°。结论　胸腰椎结核病灶清除椎间植骨融合 ,能恢复脊柱的稳定性 ,骨融合率高 ,可纠正脊柱后凸畸形 。Objective To evaluate the curative effect of radicl debridement and anterior spinal fusion on the treatment of tuberculosis of thoracic vertebrae and lumbar vertebrae.Methods Among 15 cases,the involved vertebral bodies were at 2 segment in 10 patients,at 3 segments in 5 patients.The kyphosis was 13～45 degrees with a mean kyphosis of 30 degrees.Radical debridement and spinal fusion with strut graft were used in all patients.In addition,the spine was instrumented in 2 cases for further stabilization.Strut grafts consist of ribs in 5 cases,iliums in 10 cases.Results The period of follow up lasted from 1 to 8 years with a mean time of 4 years.92% of the patients had good results and 97% had solid bony fusion.The kyphosis deformity was 15～20 degrees with an average of 14 degrees.Conclusion The radical debridement and anterior fusion with strut graft is effective for restoring the local anatomical alignment and the stability with high rate of solid bony fusion and lower rate of postoperative low back pain.
