拉萨市区藏族中老年人群高血压病患者的血压与心电图变化分析

摘　　要：目的 :对拉萨市区藏族中老年人群高血压患者采用心电图明尼苏达编码的方法 ,分析高血压患者血压与心电图变化规律 ;方法 :对拉萨市区 1 974名 40岁以上的确诊高血压病患者进行血压测定 ,同时进行心电图检查 ,心电图采用明尼苏达编码的方法进行统计 ,对编码中常见异常指标与血压进行比较 ;结果 :①拉萨市区藏族中老年高血压人群中 90 .3 2 %血压未控制到正常范围水平 ;在病人中三分之一以上的高血压患者有心电图编码的异常 ,编码异常率为 44 .53 % ;较平原地区高 ;②高血压患者中异常编码的主要类型为T波改变及ST段的压低 ,有 1 .42 %的患者出现Q波改变 ,心律失常的发生率为5.2 2 % ,随着高血压患者年龄的增加 ,异常编码率明显增高 ;③收缩压心电图异常明显相关 ;结论 :西藏拉萨市区藏族中老年高血压人群中 ,心电图异常编码率较高 ,异常编码率与收缩压和脉压水平有关 ,以T波改变和ST段压低多见。随着年龄增长及收缩压升高 ,异常心电图编码率明显增高 ,必须对高血压患者进行干预 ,采取措施。Objective:To study the changes of ECG and BP Using Minnesota code in mid-elderly population with hypertension in the urban and rural of Lhasa City.Methods:Blood pressure and ECG in 1974 cases (40 years and over) with hypertension were examined and the data was analyzed statistically by means of Minnesota code.The relationship between the abnormal code and blood pressure was researched.Results:① 90.32% of the patients with high blood pressure in the urban and rural had a beyond the normal range of blood pressure.Over one third of all patients had abnormal code and the code abnormal frequency (44.53%) was higher in mid-elderly patients at high altitude than that in plain area.②T wave changes and ST segment depression were the most common abnormalities.Pathologic Q wave and arrhythmia codes occurred only 1.42% and 5.22% respectively.The abnormalities of code were increased with the increasing age.③systolic pressure showed obviously positive correlation with ECG abnormalities.Conclusion:The higher abnormal codes of ECG,specially ST-T changes,in the hypertension in the mid-elderly population in the urban and rural areas of Lhasa city were the most common and increase with the rise of systolic pressure and the growth of the age. The therapeutic intervention measures should be taken to help the patients with hypertension in high altitude.
