封闭式体外浓缩自体腹水回输治疗大量顽固性腹水的临床观察

摘　　要：目的:观察封闭式体外浓缩自体腹水回输治疗大量顽固性腹水的疗效。方法:将14例大量腹水患者分成实验组(8例)和对照组(6例),实验组借助血透设备,直接建立体外腹水循环,采用体外浓缩自体腹水回输治疗方法。对照组采用体内腹水浓缩回输方法。同时监测腹水浓缩前后体重、腹围、血压、脉搏、血浆蛋白测定。结果:①体外浓缩法放出腹水量,超滤量,浓缩回输量明显高于体内浓缩法(P<0.05);②血浆蛋白回输率亦明显高于体内浓缩法(P<0.05);③无感染,心力衰竭等副作用。结论:封闭式体外浓缩自体腹水回输对大量顽固性腹水有明显的治疗作用,该方法明显优于体内浓缩法,可迅速缓解症状,而且安全可靠副作用小。AIM:Toobservetheefficacyofclosedextracorporealconcentrationandre-infusionofascitesintreatmentofpatientswithtenseascites.METHODS:Fourteenpatientswithtenseasciteswererandomlydividedintoexperimentgroup(n=8)andcontrolgroup(n=6).Experimentgroupadoptedtheequipmentofhemodialysistoestablishascitecirculationinvitrodirectlyforultrafiltratingandconcentratingascitessothatasciticfluidwasreinfusedintothebodyviathevein.Controlgroupwasappliedinteracorporalconcentrationtotreattenseasciticpatients.Andmeanwhilebloodpressure,thelevelofabdmencircle,weightandalbuminhemodialysisweremonitoredbeforeandaftertreatment.RESUTLS:①Theamountofasciticfluid,ultrafiltrationandreinfusiondischargedfromabdomenandextracorporalcirculationinextracorporalconcentrationwassignificantlyhigherthanthoseofinteracorporalconcen-tration(P<0.05).②TherateofplasmaAlbumreinfusedintothebodyinextracorporalwaywasalsosignificantlyhigherthanthatininteracorporalway(P<0.05).③Infection,heartfailureandotherilleffectswerenotfound.
