摘　　要：目的 探讨Madigan手术方法在治疗前列腺增生症中的应用。方法 回顾性分析48例前列腺增生症患者应用Madigan术式治疗的临床资料。结果 手术时间70-150(平均98)min,术中出血量200-400 mL。术中尿道损伤20例,切除前列腺组织25-100(平均58)g。术后继发性出血2例,再置管冲洗,痊愈出院。术后随访3～18月,无尿道狭窄、尿失禁。结论 Madigan术式合乎解剖生理特点,术后并发症少,住院时间短,保持了生理性排尿和正常射精功能。但手术视野小,尤其是肥胖患者,易损伤前列腺包膜前静脉丛,术中出血较多。尿道易损伤,有中叶增生或伴有膀胱结石,膀胱肿瘤者需再次切开膀胱处理。AIM To study the applyment of Madigan prostatectomy in benign prostatic hyperplasia (BPH). METHODS A retrospective analysis was made for 48 cases with BPH undergoing Madigan prostatectomy. RESULTS The duration of surgery was 70/50 minute (with an average of 98 minute). The volume of hemorrhage was 200-400mL, urethra was injured in 20 cases, and prostate tssue was resected for 25-100g, (with an average of 98g ), The patients had been followed up for 3-18month. No urethral stricture or incontinence occurred. CONLUSION Madigan prostatectomy had less complication, less time of hospitalization. It kept normal function of miction and ejacutation, The disadvantage include small visual field of operation. Especially in fat patients, veniplex in front of prostate envelope and urethra were injured easily. The patients with middle lobe hyperplasia or cystolithiasis or tumour needed section of bladder again.
