异丙酚对低温体外循环期间血糖及血乳酸的影响

摘　　要：目的 :观察异丙酚对心内直视手术病人低温心肺转流 (CPB)期间血糖及血乳酸的影响。方法 :30例心内直视手术病人随机分为对照组 (A组 ,n =15 )和异丙酚组 (B组 ,n =15 )。两组麻醉诱导方法相同。两组均用芬太尼、咪唑安定、维库溴铵及异氟醚维持麻醉。B组在CPB期间持续静脉注入异丙酚 8mg/kg·h。分别于麻醉诱导后 (T1)、CPB前 (T2 )、停机前 (T3 )和手术结束前 (T4)测定桡动脉血糖及血乳酸浓度。结果 :异丙酚组各时点血糖、血乳酸浓度均无显著变化 ;对照组在T3 、T4两时点血糖及血乳酸浓度均较T1、T2 两时点明显升高 (P <0 .0 5 )。结论 :低温CPB期间予异丙酚 6～ 8mg/kg·h持续静脉泵注能维持血糖及血乳酸浓度正常。Objective:To investigate the influence of propofol on blood concentration of glucose and lactate during hypothermic cardiopulmonary bypass (CBP).Methods:Thirty patients undergoing cardiosurgery under CPB were randomly divided into control group(group A,n=15) and propofol group(group B,n=15).Anesthesia induction of the two groups was the same .The drugs for maintenance of the two groups were fentanyl ,midazolum,vecuronium and isoflurane .In group B,propofol 6～8mg/kg·h was infused intravenously during CPB .Radial artery blood samples were taden for the measurements of blood glucose and lactate after induction (T1),before CPB(T2),at the end of CPB(T3) and at the end of the operation(T4) respectively. Results:Blood concentration of glucose and lactate was not significantly changed in propofol group.In control group,blood concentration of glucose and lactate significantly increased at the end of CPB and at the end of the operation (P <0.05).Conclusion:Infusion of propofol at a rate of 6～8mg/kg·h intravenously during CPB can keep blood concentration of glucose and lactate normal.
