摘 要:目的 :应用SPECT观察干细胞移植后犬急性梗死心肌内血流灌注及心肌细胞活性的改变情况。方法 :取12只成年犬骨骼肌 ,分离干细胞并培养、传代、标记。将DAPI标记的干细胞 ,自左冠状动脉前降支注入急性心肌梗死动物模型缺血心肌中。 2 ,4 ,8周后自前肢浅静脉注射99Tcm-MIBI 2 0mCi,6 0min后显像 ,进行断层采集 ,范围从右前斜 4 5°到左后斜 4 5°,共 180°。结果 :实验前各实验动物SPECT心肌断层显像 ,所有断层均见不到显影的放射性缺损 ;实验组SPECT心肌断层显像与对照组比较 ,所有断层均见不到显影的放射性缺损 ,只可见到显影的稀疏区域 ,心肌血流灌注及细胞活性得到明显改善 ;实验组心肌灌注显像评分与对照组比较差异有显著性 (P <0 .0 5 )。结论 :干细胞移植可使缺血心肌的血流灌注及细胞活性得到明显改善 ;门控99Tcm-MIBISPECT心肌灌注显像操作简便 ,安全无创 ,结果准确可靠 ,能直观地反映缺血心肌内细胞活性及血流灌注的情况。Objective:The aim of the present study was to observe the changes of myocardial perfusion imaging and the viability of iscehmic myocardium after stem cell implantation with myocardial single photon emission computed tomography (SPECT) with Tc-MIBI.Methods:Stem cells isolated from the skeletal muscle of 6 adult canines are labeled with 4', 6-dimidino-2-phenylindone(DAPI), which bands to DNA and to the protein tubulin to form a fluorescent complex. A ligation of left anterodecendant artery (LAD) was carried out on a canine model to induce a well-delineated transmural lesion. Thereafter, stem cells (experimental groups n=6) orculture media (control groups n=6) were infused into ventricular myocardium ofisogenic canine through left anterodecendant artery(LAD). Two, four, and eight weeks after cell implantation, regional Tc-MIBI uptake for each myocardial segment from SPECT and myocardial viability was determined.Results:Tc-MIBI uptake for each myocardial segment from SPECT and myocardial viability were normal in canine before experiment, and the perfusion defects and rarefaction in myocardiurn were not found. In every experimental group, perfusion defects were not found, but there was radioactivity rarefaction in every experimental group. There were sigaificant improvement in the blood perfusion and cardiomyotes activity of ischemic myocardium. There were significant differences in the marks of perfusion defects between the experimental and control groups ( P <0.05).Conclusion:Stem cells implantation could improve the blood perfusion and cardiomyocytes activity. Our data suggest that myocardial perfusion SPECT imaging may be invasive, safe, simple in examining the blood peffusion and cardiomyotes activity of ischemic myocardium.
