苦参碱联合α-1b干扰素治疗慢性乙型肝炎临床观察

摘　　要：目的 :观察苦参碱注射液和α 1b干扰素 (IFNα 1b)联合用药对慢性乙型肝炎的疗效。方法 :将 10 2例慢性乙型肝炎患者分为治疗组 5 6例和对照组 4 6例。治疗组采用α 1b干扰素 5 0 0万u ,三角肌注射 ,每周 3次 ,苦参碱注射液 15 0mg加入 10 0 g/L葡萄糖注射液 2 5 0ml中静滴 ,每日 1次 ;对照组单用α 1b干扰素 5 0 0万u ,三角肌注射 ,每周 3次。两组疗程均为 16周。治疗前后各检测肝功能、HBeAg及HBV DNA。结果 :治疗组谷氨酸转氨酶 (ALT)、天冬氨酸转氨酶 (AST)复常率显著高于对照组 (P <0 .0 1) ;治疗组疗程结束时HBeAg、HBV DNA阴转率分别为 5 3.6 %(30 /5 6 )、 6 0 .7% (34/5 6 ) ,对照组分别为 39.0 % (18/4 6 )、 4 1.7% (19/4 6 ) ,两组比较 ,差异有显著性 (P <0 .0 5 ) ;治疗组治疗期间 ,头痛、发热、肢体酸软、白细胞减少等不良反应发生率显著低于对照组。结论 :苦参碱联合IFNα 1b可提高对慢性乙型肝炎的治疗效果 。Objective:To study the curative effect of Matrini Injection and interferon-α-lb (IFNα-1b) in treating chronic hepatitis B. Method:One hundred and two cases of chronic hepatitis B were divided into treatment group and control group ( 56 cases for treatment group and 46 cases for control group).Trigonum muscle injec-tion with 5×10 6 u IFNα-1b was given three times every week,and drip infusion technique by adding 150 mg Matrini Injection into 100 g/L dextrose solution was adopted once daily for 16 weeks in treatment group,while trigonum muscle injection with 5×10 6 u IFNα-1b only in control group. Liver function,HBeAg and HBV-DNA were tested before and after treatment.Result:In treatment group, The alanine aminotransferase and aspartate aminotransferase recovery rates increased compared with those in control group (P<0.01).The negatively transforming rate of HBeAg and HBV-DNA in treatment group was 53.6%(30/56) and 60.7%(34/56) respectively, which was significantly higher than the 39.0%(18/46) and 41.7%(19/46) in control group (P<0.05).The side-effects in treatment group were fewer than those in control group. Conclusion: The combined therapy of Matrini Injection and IFNα-1b maybe improve the curative effects on chronic hepatitis B,and reduce the side-effects.
