李氏清暑益气汤加减治疗多发性神经根炎32例疗效观察及肌电图分析

摘  要:目的:观察李氏清暑益气汤加减治疗多发性神经根炎的临床疗效及对肌电图F波传导速度的影响。方法:对32例多发性神经根炎患者采用李氏清暑益气汤(由黄芪、党参、白术、苍术、麦冬、神曲、黄柏、泽泻、当归、炙甘草、升麻、陈皮、青皮、五味子、葛根组成)辨证加减治疗,同时用肌电图观察治疗前后F波传导速度的改变。结果:治愈15例,好转12例,无效5例,总有效率为84.4%。随着病情的好转,尺神经和腓神经F波传导速度均明显提高,治疗前后比较,差异有非常显著性意义(P<0.001)。结论:多发性神经根炎的主要病机为脾虚湿困。补中益气、清热化湿、强筋健肌疗法可以使神经根炎症消除,节段性髓鞘脱失较好地修复,传导功能改善。Objective: To observe the curative effect of modified Li' s Qingshu Tang(LQT) for multiple radiculoneuritis(MR) and its influence on conduction velocity of electromyogram F wave. Methods: 32 cases of MR were treated by LQT( composed of Radix Astragali, Radix Codonopsis, Rhizoma Atractylodis Macrocephalae, Rhizoma A-tractylodis, Radix Ophiopogonis, Massa Medicata Fermentata, Cortex Phellodendri, Rhizoma Alismatie>, Radix Angelicae Sinensis, Radix Glycyrrhizae Preparata, Rhizoma Cimicifugae, Pericarpium Citri Reticulatae, Pericarpium Citri Reticulatae Viride, Fructus Schisan-drae arid Radix Puerariae) modified with syndrome differentiation, and the conduction velocity of electromyogram F wave were observed simultaneously before and after treatment. Results: Among them, 15 cases were cured, 12 cases improved and 5 cases ineffective, the total effective rate being 84. 4% . The conduction velocity of F wave of ul-nar nerve and peroneal nerve was markedly improved after treatment ( P < 0. 001). Conclusion: The main pathogensis of MR is spleen deficiency leading to stagnation of damp, so that the therapeutical principle of invigorating middle ~ jiao, benefiting qi, clearing heat and damp, and strengthening the tendon and muscle can alleviate radicu-loneuritis, repair the segmental demyelination and improve the conducting function.
