小儿细菌感染时对胰岛素样生长因子-1及其结合蛋白的影响

摘　　要：目的　探讨小儿细菌感染时血清胰岛素样生长因子 (IGF 1)和胰岛素样生长因子结合蛋白 3(IGFBP 3)水平的变化及其临床意义。方法　用放射免疫法和免疫放射法检测血中的IGF 1和IGBP 3。感染组患儿发病极期 30例、恢复期 10例 ,正常对照组 30例。用酶联免疫法测肿瘤坏死因子 (TNF α)。结果　(1)在感染极期 ,血清IGF 1(45 0± 14 8) μg/ml及IGFBP 3(12 85 9± 6 0 8 0 ) μg/ml水平明显降低 ,较感染恢复期 (P <0 0 5 )和正常对照组 (P <0 0 0 1) ,差异均有显著性意义 ;在感染恢复期 ,血清IGF 1及IGFBP 3水平已近正常 ,与对照组比较 ,差异无显著性意义 ,且二者的改变存在明显正相关。 (2 )在感染极期 ,血TNF α (32 9± 6 6 )pg/ml水平明显升高 ,较感染恢复期 (P <0 0 0 1)和正常对照组 (P <0 0 0 1) ,差异均有显著性意义 ;在感染恢复期血TNF α水平已近正常 ,较对照组 (P =0 2 78) ,差异无显著性意义。无论在感染极期 ,还是在感染恢复期 ,血IGF 1、IGFBP 3与TNF α水平均无相关性。结论　 (1)小儿细菌感染发病极期 ,血清IGF 1、IGFBP 3水平显著降低 ,恢复期恢复正常 ,二者的改变具有相关性。 (2 )血清TNF α浓度在感染极期升高 ,恢复期降至正常 ,但未发现与IGF 1、IGFBPObjective To explore changes of serum levels of IGF-1 and IGFBP-3 during bacterial infection in children.Methods Serum levels of IGF-1 and IGFBP-3 were measured respectively by radioimmunoassay and immunoradiometric assay.Infectious group included 30 children in acute phase and 10 cases in convalescent phase.Another thirty healthy children served as control group.Serum levels of TNF-α was measured by immunoenzymometric assay.Results (1)Serum levels of IGF-1(45.0±14.8)μg/ml and IGFBP-3(1285.9±608.0)μg/ml in acute phase in infections group were markedly decreased.There were significantly different,compared with those of convalescent(P<0.05) and control group(P<0.001).Serum levels of IGF-1 and IGFBP-3 in convalescent group already returned to normal,and there was no significant differences compared with those of control group(P=0.848,0.544).There existed significant positive correlation between serum levels of IGF-1 and LGFBP-3.(2)Serum levels of TNF-α(32.9±6.7)pg/ml in acute phase in infectious group had obviously elevated,and there was significantly different compared with that of convalescent group(P<0.001)and control(P<0.001)group.But serum levels in convalescent group almost returned to normal,and there was no significant difference compared with that of control group.Conclusion Neither in acute phase,nor in convalescent,there were correlations among serum levels of TNF-α and IGF-1 and IGFBP-3.
