摘 要:目的:探讨HBV DNA定量检测在乙型肝炎病毒宫内感染监测中的临床意义。方法:将134例HBsAg(+)孕妇及其所生婴儿作为研究对象,按孕妇HBsAg定性结果分为阳性组和阴性组。孕妇产时取血;新生儿出生2小时内取血,并注射高效价乙肝免疫球蛋白(HBIG)及乙肝疫苗,于7、12月龄时再分别取血留检。采用英国罗氏公司生产的Light Cycler Quantification直接进行ABV DNA定量测定,同时与HBsAg和HBV DNA定性检测结果比较。结果:HBsAg阴性组中依然有17.0%的病例HBV DNA定性阳性,且HBV DNA定量均值与阳性组比较无统计学差异(P>0.05);新生儿血清HBV DNA阳性病例的母亲血清HBV DNA定量均值明显高于阴性病例(P<0.01);HBVDNA阳性的新生儿母婴阻断成功率明显降低(P<0.01)。结论:用分子生物学技术证明孕期HBV DNA定量检测是监测HBV宫内感染程度的可靠方法。Objective: To explore the important role of serum hepatitis B virus DNA quantitative determination in monitoring serum HBV intrauterine infection. Methods: Serum HBV DNA load were determined by Light Cycler Quantification made in ROCH company and to compare with HBV Mark and HBV DNA qualitative determination. Results: There were 17.0% cases positive in serum HBV DNA qualitative determination in serum HBeAg negative pregnant women and these pregnant women HBV DNA quantitative mean were not significantly related to other HBeAg negative pregnant women, P>0.05. Besides, pregnant women serum HBV DNA quantitative mean in their newboms serum HBV DNA positive increased significantly than other pregnant women of newboms negative, P<0.01. The rate of HBV female-infunt block is decreased in newborns serum HBV DNA Positiue, P<0.01. Conclusion : By molecular biology technique, we concluded that pregnant women HBV DNA quantitative determination was reliable means to monitor serum HBV intrauterine transmission.
