摘  要:目的 :探讨呼吸道合胞病毒肺炎 (RSVP)患儿外周血白细胞表面 CD11b、CD5 4在发病中的作用及其与病情的关系。方法 :采用全血直接免疫荧光流式细胞术检测 30例 RSVP患儿 (轻型 2 0例、重型 10例 )及 2 0例正常小儿外周血白细胞表面粘附分子 (AMS) CD11b、CD5 4的表达。结果 :(1) RSVP组与对照组比较 :淋巴细胞 (L )、单核细胞 (M)多形核细胞(PMN)表面 CD11b、CD5 4的阳性细胞百分率 (PPC)和 /或平均荧光强度 (MFI)普遍上调 (P <0 .0 5或 P <0 .0 1) ,其中以 M的 AMS上调最为显著 ;(2 )重型 RSVP组与轻型组比较 :L、M表面 CD11b 的 PPC和 /或 MFI表达上调 ,3类白细胞表面CD5 4的 PPC和 MFI表达均显著上调 (P <0 .0 5或 P <0 .0 1)。结论 :(1) AMS在 RSVP的发病中发挥了重要作用 ;(2 )在 3类白细胞表面 CD5 4表达均同时上调的水平有可能作为判断肺炎病情程度的免疫学指标 ;(3)本研究结果可能为肺炎的抗粘附疗法提供一些依据。Objective: To explore the expression of AMS including CD11b and CD54 on peripheral blood leukocyte surface (PBLS) in children with respiratory syncytial virus pneumonia (RSVP).Methods: The whole blood immunofluorescence flow cytometry method was used to detect the expression of CD11b and CD54 on the surfaces of lymphocytes(L),monocytes(M) and polymorphnuclear leukocytes(PMN) in 30 children with RSVP (20 severe RSVP and 10 mild and 20 control children . Results: (1) PPC and /or MFI of CD11b and CD54 on PBLS in children with RSVP were generally higher than those in control group, and AMS on M was the most outstanding (P<0.05 or P<0.01). (2) There were higher PPC and /or MFI of CD11b and CD54 on L and M in severe RSVP group than those in mild RSVP group ,and PPC and MFI of CD54 on the three kinds of leukocytes were significantly increased (P<0.05 or P<0.01) . Conclusions: (1) CD11b and CD54 on PBLS might play an important role in RSVP (2) The higher expression of CD54 on PBLS might be an immmunologic index suggesting the condition of RSVP.
