摘  要:目的 :以小支气管平滑肌厚度、粘膜网状基底膜厚度作为气道重塑指标 ,观察支气管哮喘患者支气管细胞外基质变化 ;了解哮喘患者血清、支气管肺泡灌洗液中细胞外基质相关因子的变化。方法 :对 12例哮喘患者和 10例非哮喘对照者经纤支镜支气管粘膜活检 ,测量支气管平滑肌、粘膜网状基底膜厚度 ,测定血清及支气管肺泡灌洗液 (BALF)中细胞外基质相关因子 :转化生长因子 β1(TGFβ1)、γ干扰素 (IFN -γ)、白介素 4 (IL - 4)、白介素 5 (IL - 5 )、III型前胶原 (PCIII)的含量。用t检验比较两组小支气管平滑肌及粘膜网状基底膜厚度以及上述细胞因子含量。结果 :哮喘患者小支气管平滑肌厚度、粘膜网状基底膜厚度明显高于对照组 ,差异有显著性。哮喘患者血清和BALF中TGFβ1、IL - 4、IL - 5、PCIII含量明显高于对照组 ,IFN -γ含量明显低于对照组 ,差异均有显著性。结论 :哮喘患者小支气管平滑肌厚度、粘膜网状基底膜厚度均存在不同程度增厚 ,血清和BALF中某些细胞外基质相关因子存在显著变化 ,可能存在Th1/Th2失衡。Objective: To compare the thickness of small bronchial smooth muscule and subepithelial reticular basement membrane (BM) between asthma patients and non-asthma patients. To compare the level of the cytokines related to extracellular matrix between asthma patients and non-asthma patients. Methods: Fiberoptic bronchoscopy with bronchial biopsy was performed on 12 asthma patients and 10 non-asthma patients. The thickness of small bronchial smooth muscule and subepithelial reticular basement membrane (BM) were measured. Levels of transforming growth factor β1(TGFβ1),interferon-γ(IFN-γ),interleukin-4(IL-4), interleukin-5(IL-5),PCIII in the serum and BALF were measured. Use t-test as the statistical method. Results: The mean thickness of small bronchial smooth muscule and BM were signicantly higher in asthma patients than in non-asthma patients. The levels of TGFβ1, IL-4, IL-5,PCIII in the serum and BALF were significantly higher in asthma patients than in non-asthma patients; while The levels of IFN-γ were significantly lower in asthma patients than in non-asthma patients. Conclusions: Airway remodeling is a characteristic pathologic finding of asthma, there may be the unbalance between the Th1 and Th2 cytokines in asthma.
