降压益肾颗粒治疗高血压病早期肾损害临床观察

摘　　要：目的　观察降压益肾颗粒对高血压病早期肾损害的疗效。方法　应用动态血压技术评定 30例高血压病伴有尿 β2-微球蛋白 (β2 -MG)或微量白蛋白 (mAlb)增高的患者服用降压益肾颗粒治疗前后的降压疗效 ,并测定治疗前后的尿 β2-MG和尿mAlb ,用开搏通做对照。结果　降压益肾颗粒治疗组 2 4hSBP、2 4hDBP、SBPV、DBPV、SBP -L、DBP -L、尿 β2 -MG、尿mAlb较治疗前明显下降 (P <0 .0 1) ,De -SBP、De -DBP明显升高 (P <0 .0 1) ;开搏通对照组 2 4hSBP、2 4hDBP、SBP-L、DBP -L、De -SBP、De -DBP、尿 β2 -MG、尿mAlb显著低于治疗前 (P <0 .0 1或P <0 .0 5 ) ,SBPV、DBPV无明显变化。治疗后对照组 2 4hSBP、2 4hDBP、De -SBP、De -DBP明显低于治疗组 (P <0 .0 5或P <0 .0 1) ;治疗组SBPV、DBPV、SBP -L、DBP -L、尿 β2 -MG较对照组下降明显 (P <0 .0 5或P <0 .0 1)。结论　降压益肾颗粒对高血压病早期肾损害有明确的降压和保护肾脏作用 ,其保护肾脏的作用优于开搏通 ,机理可能与减小血压变异、增大夜间血压下降率、降低血压负荷、一定程度的恢复血压昼夜节律有关。OBJECTIVE To observe the therapeutic effect of Jiangya Yishen Granule in the treatment of renal lesion in the early stage of hypertension. METHOD Dynamic blood pressure technique was used to evaluate the therapeutic effect of Jiangya Yishen Granule in treating 30 hypertensive patients with increase of β 2-MG or mAlb. RESULT In the treatment group, the levels of 24h SBP, 24h DBP, SBVP, DBVP, SBP-L, DBP-L, urineβ 2-MG and urine mAlb dropped markedly (P<0.01), and De-SBP, De-DBP rose markedly (P<0.01). In the control group treated with Kaibotong, the levels of 24h SBP, 24h DBP, SBP-L, DBP-L, De-SBP, De-DBP, urineβ2-MG and urine mAlb were markedly lower than those before the treatment (P<0.01 or P<0.05) and there are no obvious changes in the levels of SBVP and DBVP. After the treatment, the levels of 24h SBP, 24h DBP, De-SBP and De-DBP in the control group were markedly lower than those in the treatment group (P<0.05 or P<0.01) and the level of SBPV, DBPV, SBP-L, DBP-L, and urineβ 2-MG in the treatment group were markedly lower than those in the control group (P<0.05). CONCLUSION Jiangya Yishen Granule has marked effect in lowering blood pressure and protecting the kidney, the mechanism of which is probably related to the decrease of blood pressure variation, the increase of the dropping rate of blood pressure during the night, the decrease of blood pressure load, and the restoration of the blood pressure rhythm.
