二尖瓣替换保留二尖瓣瓣下结构方法研究

摘　　要：目的　研究二尖瓣替换时保留二尖瓣瓣下结构的方式 ,提高疗效。方法　 75例二尖瓣病变行瓣膜置换时全部或部分保留二尖瓣瓣下结构 ,用翻转、移植、修剪、部分保留等方法。合并其余瓣膜病变时同期处理。结果　术后早期死亡 1例 ,病死率 1 3%。随访 0 3～ 7年 ,平均 3 6年 ,无晚期死亡。临床心功能Ⅰ级 6 8例 ,Ⅱ级 6例。结论　行二尖瓣替换时保留二尖瓣瓣下结构要掌握好适应症 ,根据病理改变选择方法 ,可取得良好疗效。Objective Study the method of preservation of mitral complex in mitral valve replacement. Methods 75 cases were studied. The method include:reverse,tansplant,trim,and partial preservation. Results Early postoperative death was 1 case(death rate 1.3%) . The follow-up ranged from 0.3 to 7 years, average 3.6 years. No late postoperative death. Heart function(NYHA):68 cases in I class,6 cases in Ⅱ class. Conclusion Preservation of the mitral complex in mitral valve replacement should know well the indications. Method selection should accord with pathology of the mitral valve.Better effect is expected.
