椎板截骨腰椎管扩大成形术的临床应用

摘　　要：目的:总结椎板截骨腰椎管扩大成形术的临床应用效果及方法。自1995年8月～2001年6月共施行本手术33例,治疗腰椎间盘突出症并腰椎管狭窄症和腰椎管狭窄症,该术式是切除内侧部分关节突、扩大侧隐窝,将截下的椎板再植、椎管后壁稍后移,扩大椎管。结果:33例中27例获得1.5～7年随访,优良率达88.9％。结论:适用于确需行椎管彻底减压时的一种好术式。Objective: To summarize the effect of clinical application and method in the treatment of lamina osteotomy extensive plasty of the lumbar canal. Method: From Aug. 1995 to Jun. 2001, 33 cases were treated with this method to cure protrusion of intervertebral disc and lumbar spinal stenosis, lumbar spinal stenotic myelopathy as well. The method was to cut off medial parts of the articular process, enlarge the lateral recess, reconstruct the lamina of verterbral arch, which had been cut off, backward extending the posterior portion of spinal arch a bit, to enlarge the vertebral canal. Result: Among the 33 cases, 27 were followed up from 1.5 to 7 years. The excellent rate reaches 88.9% . Conclusion: It is an effective measure to treat the case which is needed accept a thorough lumbar canal decompression.
