^(18)F-FDG符合线路显像与^(67)Ga显像诊断结节病的对比研究

摘　　要：目的　对比分析1 8F 脱氧葡萄糖 (FDG)符合线路显像 (FDG显像 )和6 7Ga显像对结节病的诊断和疗效监测的价值。方法　 17例结节病患者行1 8F FDG显像 ,其中 11例行6 7Ga显像。以目测法判断纵隔和肺门淋巴结放射性摄取程度 ,明显超过周围肺组织本底水平者为阳性。结果　 11例未治疗的结节病患者1 8F FDG显像均阳性 ,其中 7例行6 7Ga显像者影像特征与1 8F FDG显像相似 ;1 8F FDG、6 7Ga显像均阳性的 2例患者 ,治疗后X线片示病灶吸收 ,再次显像均为阴性 ;1 8F FDG显像本底较低且显像效果较好。 3例已治疗者X线片示病灶吸收 ,1 8F FDG显像均阴性 ,其中 2例行6 7Ga显像也阴性。 3例临床诊断为结节病者 ,1 8F FDG显像阳性 ,其中 2例6 7Ga显像也阳性 (1例治疗后 2种显像仍呈阳性 ,而X线片示纵隔肺门肿大淋巴结缩小 )。结论　1 8F FDG符合线路显像比6 7Ga显像效果好 ,对结节病诊断和治疗效果监测有较好的应用价值。Objective To compare 18F-fluorodeoxyglucose (FDG) coincidence imaging and 67Ga imaging in diagnosis and therapeutic monitoring in sarcoidosis. Methods Seventeen patients underwent 18F-FDG coincidence imaging, among them 11 cases received 67Ga imaging in addition. The uptakes of the tracer in the mediastinal and pulmonary hilar lymphnodes were judged by visual method with the uptakes in the lymphnodes surpassing markedly that in the surrounding lung tissue to be regarded as positive. Results Eleven sarcoidosis patients without receiving any therapy had positive 18F-FDG coincidence images, among them 7 cases received 67Ga imaging in addition and obtained similar images as with 18F-FDG. The radiocounts of the background on the 18F-FDG images was lower yet with a better image quality when compared with 67Ga images. The X-ray films showed that the lesions were absorbed with negative 18F-FDG images in 3 cases after treatment, among them 2 cases with negative 67Ga images as well. Two cases with positive results from these 2 modalities received treatment and the subsequent X-ray examination demonstrated that the lesions were absorbed and the images from these 2 modalities changed to negative, too. Three cases with clinically diagnosed sarcoidosis got positive 18F-FDG images, among them 2 cases also received 67Ga imaging and had positive results, too; the image quality showed that the 18F-FDG was better than 67Ga. The X-ray film showed that the pulmonary hilar lymphnodes shrank in 1 patient after treatment but the images of these 2 modalities remained positive. Conclusion The study shows that 18F-FDG is superior to 67Ga in the diagnosis and therapeutic monitoring of sarcoidosis.
