血液透析相关低血压的原因及防治对策

摘　　要：目的　探讨慢性肾功能衰竭血液透析相关低血压的原因及防治对策。方法　选择 2 9例血透低血压患者 (低血压组 )与 2 5例无低血压患者 (对照组 )进行相关因素比较。结果　①低血压组与对照组相比透析间期体重增长率、超滤量 (UFV)、超滤率 (UFR)较高 (P <0 .0 1) ,而血钠、血白蛋白 (Alb)、血红蛋白较低 (P <0 .0 1) ,血肌酐 (Cr)差异不显著 ;②低血压组室间隔、左室后壁增厚程度及左室顺应性减低程度较对照组明显 ;③监控体重增长、调整透析模式、采取综合治疗可降低透析低血压的发生率。结论　血液透析相关低血压是多因素综合作用的结果。分析具体病情、采取针对性措施、关注高发人群 。Objective To try to find out the causes and the therapy to hemodialytic hypotension in the patients with chronic renal failure (CRF).Methods 29 CRF patients with hemodialytic hypotersion (hypotension group) were compared with 25 CRF patients without hemodialytic hypotension (control group).Results ①Hemodialytic weight gain rate UFV and UFR in hypotension group were higher and serum Na,Alb and Hb in hypotension group were lower than those in control group ( P <0.01 ).There was no significant difference in Cr between the two groups;②Interventricular septum thickness,left ventricular posterior wall thickness and left ventricular diastolic dysfunction in hypotension group were higher than those in control group;③There were some ways that could decrease hemolytic hypotension incidence,they were to control hemodialytic weight gain and to apply new hemodialytic technology,ect.Conclusions Hemodialytic hypotension can be caused by many factors.We must make concrete analysis of concrete conditions and give the patients suitable treatment.
