四肢一次性切痂自体微粒皮移植术治疗大面积深度烧伤

摘　　要：目的　寻找大面积深度烧伤的有效治疗方法。方法　 2 4例大面积深度烧伤于伤后 3～ 6d行四肢一次性切痂、自体微粒皮移植术修复创面。结果　① 2 4例大面积深度烧伤患者有 2 0例存活 ,其中微粒皮成活及覆盖创面 90 %以上者 12例、70 %～ 90 %者 8例。② 5～ 7d后 ,异体皮表皮层出现疱状变性 ;2周左右 ,异体真皮出现黑色斑点 ,4～ 6周形成痂皮脱落 ,其下为新生表皮覆盖。③随访 2年见愈合后的创面平整、光滑、有弹性。结论　四肢一次性切痂微粒皮移植术治疗大面积深度烧伤切实可行。Objective To explore effective treatment of extensively deep burns. Methods The devitalized tissue was excised 3～6 days after burns.Wounds were covered by the combination of micro skin autograft and allogeneic skin graft in 24 cases. Results ①The 20 patients survived. The wounds were effectively covered beyond 90% in 12 cases,in between of 70%～90% in 8 cases. ②From 5～7 days,there was vesicular degeneration of allo epidermis. From 14 days, there were black spots of allo dermis. From 4～6 weeks,the sloughed allo dermis as a crust was separated from the healed new epidermis. ③The healing wounds were soft and smooth. Conclusion The combination of micro skin autograft and allogenic skin graft after extensive eschar excision in one operation is effective and feasible treatment for extensively deep burns.
